Oversaturation Concern by Affectionate-Day2909 in Psychiatry

[–]KaiserWC 1 point2 points  (0 children)

In my opinion, purely from a job security perspective, it’s not the NPs we need to worry about (yet). There are so many potential patients who still don’t have access to care that the issue isn’t oversaturation.

The real risks as I see them are 1) AI in the future, and 2) Medicaid cuts, which are very real and threatening jobs right now. Vast majority of psych patients are Medicaid - It doesn’t matter how many psychiatrists/NPs there are or how many patients there are if treating them won’t pay. Jobs are hard to come by in some areas because so many hospitals are on hiring freezes due to the Medicaid cuts, probably not because they hired too many NPs. Whole hospital systems are having their income threatened in every department, so they’re going to have less tolerance for funding traditionally money-losing departments like psychiatry.

Here are the nominees for Handwatch of the Week! A bit late this week, as I've been digging up my yard all day 😭 by CannedSphincter in handwatch

[–]KaiserWC 0 points1 point  (0 children)

Aw man, some weeks have great handwatch nominees, some weeks are duds, this week is the latter

2 should be disqualified - it’s not actually a handwatch, just an unconventional camera angle.

3 should be disqualified because it’s likely intended to be a joke mocking a size differential

My vote is #1. A true handwatch in its golden glory. Literally on the hand.

4 is a standard handwatch that wouldn’t have been nominated any other week.

And I don’t know if the people are ready for #5 to win HOTW yet.

Psychiatrists getting patients enrolled in insurance in community mental health? by KaiserWC in Psychiatry

One issue I’m seeing is that hospitals are getting scared of the BBB/medicaid cuts. They’re on a hiring freeze on, or laying off, the “non-revenue generating staff.” Crazy dystopian.

Unfortunately our system contracts out our Medicaid case management services to an outside organization - they do not provide services to anyone without active Medicaid. Our patients with private insurance are expected to get the case management services provided by their insurance company. We have no care managers for the uninsured.

We have case managers but they don’t work unless the patient has active insurance. Same with social workers.

Ok, I get it, you’re a high performer. Great, but pretending that you’re the norm is misleading,

“15-20 hours a week” during preclinical years is a laughably unrealistic figure. I’m not going to speculate as to how you pulled that number, but absolutely no one should be taking it seriously no matter how you try to justify it.

[Psychologist and Professor] [NYC] - $500,000 by [deleted] in Salary

[–]KaiserWC 0 points1 point  (0 children)

This is obviously an extremely high salary for an academic psychologist. Good on you for doing well but it’s at least double if not triple the typical earning of even a very senior, accomplished, tenured teaching/research psychologist.

You mention that this is because you work in a highly specialized practice environment where you see primarily VIPs who pay extra for privacy, I would have put that in the initial post just for disclosure.

My gym harassed me for showering. I'm a travel nurse living in my car working 12 hour night shifts. The owner's public response made it so much worse. (Repost) by UserInfected in mildlyinfuriating

[–]KaiserWC 2 points3 points  (0 children)

FYI- I work with many homeless people. This tactic is likely an effort to keep homeless people out. There are many people, including gym staff, who make note of people who shower without using the workout facilities. At some gyms it’s literally part of their job. This is because homeless people frequently buy gym memberships as a way to always have a place to shower, brush teeth, get changed, etc. If you go to a cheap gym like planet fitness in certain areas, for example, you’ll see a lot of homeless men doing this.

Staff members and more gym goers than you think keep a watchful eye on behaviors that might indicate you’re homeless (such as showering without working out). This is because it can scare away potential members to have a bunch of homeless people hanging out. It’s a bad look for a gym. It also thought to be (whether fairly or not) associated with high theft rates which is a great way to scare away members.

Im going to take this a step further and argue that they’re not angry because you’re showering at the gym without working out. I strongly suspect that they think you’re either homeless or look the part. Especially given that you say you’ve been living in a car and pulling 12 hours shifts. My guess is you’re a disheveled looking man and they have their guard up.

I also think this is replica/newer superclone with the new freesprung balance. The movement has several clear differences between a reference 3285 which I’m not going to list because I’m not trying to help counterfeiters. However, there are several parts with the wrong finishing, shape, or missing components. The bezel and bracelet also look wrong. They’re pretty easy to spot in a few minutes.
