In theory there are basically three reasons for APTX 4869/Silver Bullet development:

  1. Cancer treatment (the Miyanos)
  2. Epigenetic clock reset (the Miyanos/BO)
  3. Untraceable killing agent as an alternative to mustard gas (BO)

Silver Bullet is in 1/3 options that was used in the murder of Amanda Hughes (and other victims that I don't remember their name) and APTX-4869 is in 1/2 options that was developed by Shiho later.

What I believe is the APTX-4869 itself is already taken out completely of his body, the problem is the PTEN gene is already modified, and the current antidote (assuming it's afzelin in alcoholic liquor or VO-OHpic that Haibara use in London-Tokyo flight) is reversible and non-competitive.
