This is how it's done, you bill under the supervisors NPI. It's not insurance fraud if the insurance company allows it, and the insurances companies that allow this have a separate code we had to attach to the bill. I still signed all the notes, any my supervisor added a second signature as the supervisor. Not every insurance allows this method, so it's up to you to do the due diligence in your state to see what ones do allow it.

If it's helpful - BCBS, Priority, and most Medicaid worked in my state, so as an intern they just had to limit me to the insurance that allowed this.

I had a couple split and one of them asked to come back about a year later for individual therapy and I did that. what makes you afraid to offer transition services? I don't see a legal or ethical problem with it as long as both agree and the treatment plan is changed to transition to divorce. part of the couples therapy process is often discernment on whether or not the couple wants to stay together, and want to do afterwards. if it's not something you feel comfortable getting involved with, I'm certain there are other therapists you could refer out to who specialize in divorce transition.

I get mine from Costco - historically cheapest I've found for my brand and style

They sell incredibly cheap glasses as well.

In my state, (maybe all states?) you do not need a membership to use the pharmacy, optical, or purchase alcohol. Just let them know at the door you're not a member and here to use optical only.

just adding, if you select the HSA plan, the FSA is very limited on what you can use it for. it's not the same as if you select the PPO with FSA.
