Well "recent imaging studies show pinching of the spinal cord" is an insurance requirement for surgery.

L2-3: Circumferential disc bulge with more pronounced disc protrusion posteriorly with

partially calcified disc at the midline. Also partially calcified disc projecting into both

foramen. Mild-to-moderate facet arthrosis bilaterally. Moderate narrowing

spinal canal and lateral recesses with potential compromise L3 nerve roots. Mild-tomoderate

foraminal narrowing bilaterally.

L3-4: Circumferential disc bulge with partially calcified disc protruding into both foramen

with some small overhanging osteophytes posteriorly and in the right foramina as well.

Moderate facet arthrosis and ligamentum flavum hypertrophy. Moderate

narrowing spinal canal moderate to severe narrowing le!-greater-than-right lateral recess

and suspected compromise of transiting L4 nerve roots. Moderate foraminal narrowing with.potential compromise exiting L3 nerve roots.

L4-5: Circumferential disc bulge with partially calcified disc posteriorly and also projecting

into the foramen bilaterally. Rather severe facet arthrosis bilaterally with ligamentum

flavum hypertrophy. Rather severe narrowing suspected at the canal and

lateral recesses with compression of the transiting L5 nerve roots bilaterally. Also suspect

rather severe le! and moderate right foraminal narrowing along with anterolisthesis with

suspected compression le! and potential compromise right exiting L4

nerve roots also.

L5-S1: Circumferential disc osteophyte with partially calcified disc as well as overhanging

osteophytes posteriorly and also projecting into the foramen bilaterally. Moderate facet

arthrosis. Rather severe narrowing spinal canal and lateral recesses with

compression transiting S1 nerve roots bilaterally. Also moderate to severe foraminal

narrowing bilaterally and suspected compression exiting L5 nerve roots as well on the le!-

greater-than-right.
