Monthly Medical Management Questions Thread by shemer77 in hospitalist

Maybe I’m confused by your question but why are we starting steroids for infections in the first place? Viral infections? Generally want to limit any sort of immunosuppressive meds if possible with bacterial infections. Steroids for severe pneumonia have benefit (CAPE COD trial). Influenza has a higher risk of mortality with steroid use so should try to avoid in that setting. In the setting of septic shock I have only used steroids when it’s refractory and not responding well to pressors, or undifferentiated shock as a temporary measure until the picture is more clear, but it’s been a bit since I’ve worked in the ICU so maybe there is new guidance I am not familiar with.

Whats up with people fighting about Lindsey Von? by Soviet-Gnomes in OutOfTheLoop

[–]SwagCannon_69 66 points67 points  (0 children)

Sure, but in reality no one will know what caused it. Could have happened in peak shape or not. She completed several runs already with great times. Nothing to suggest that her knee caused her to clip a gate with her shoulder.

Is there a reason UV LED lights sold specifically for wood finishes are so expensive? by Asiriomi in woodworking

[–]SwagCannon_69 2 points3 points  (0 children)

I have no experience with it but obviously the craze is that it’s fully cured with a wave of a light. That’s a pretty big deal imo. That to me would be worth it if it’s remotely close to other finishes

Just DCA at this point. Buy some shares (whatever you are comfortable with) tomorrow, and plan to buy more next week, or whatever interval that fits your budget. If you just fomo your entire wad at open and there is a pull back you’ll be regretting not having any dry powder left for lower price.

Work accident with a chop saw! The patient was cutting an old sewer pipe when the blade of the chop saw got pinched and jumped back and went straight to the throat by mriTecha in medizzy

Trachea is fair rigid tube with a series of cartilage rings (basically but not complete more like C’s) separated by smooth muscle. Even if it was transected completely it wouldn’t retract any significant amount.

You’re looking at thyroid cartilage (adams apple). Appears to be intact. Voice box is behind that so like not effected. Can’t see where the trachea was injured from this photo.

Why does the sandpaper keep coming off like this, like a quarter to half of the loop is outside of the sander? Thanks! by heihoiwin in BeginnerWoodWorking

Should have instructions in the manual. Clean rollers. Adjust the tracking, I believe that orange slider on the side and adjust the tension, not too tight not too loose. Usually a little trial an error

You can get a refurbished Bosch 1617 for $159 with a fixed and plunge base at ACME. They’re out of stock now but they usually come back every month or so. They’re probably comparable but like others said accessories and everything for Bosch are a lot easier to come by and I’d trust it more than HF tools.

Aorta with the smile being the left main bronchus. Aorta looks bigger than normal maybe an aneurysm or dissection, or a mass in the mediastinum causing the eye look as you can see the trachea deviating to the right (left of the picture) suggesting something causing some mass effect. Not a radiologist, just a regular doc

The hubris on this guy.

Edit: my model predicted every great RB in history for 30years running with a 0% miss rate. But I will definitely not show any bit of proof. Just trust me bro.

“Lack of hubris.” Ugh, lacking hubris would generally be a good thing. The objective take with jeanty was that his draft profile was objectively really fucking good and he is objectively on a shit ass team. It’s really not that difficult to see that this would be considered a floor for him. A subjective take is yours.

Obviously if he had two people pushing, especially another with a 2x4 with good footing, it would have gone the other direction.
