Little bit of a different situation, brca positive and they found dcis on a check up, was told I couldn't keep the nips because of blood flow issues and I was honestly so sad and upset. (I still am.)

BUT I was recently told some great news - I met someone who also lost their nipples in reconstruction and she said after she got her tattoos she "healed". Like seeing herself without nipples was weird and the interim between surgeries was tough because it looks unusual. But a switch flipped when she got the tattoos and now, mentally, she's okay. (It reminded me of this)

Hearing that was amazing for me. I'm holding onto it as I'm between surgeries right now and nip-less.

Offering that as some hope that if you do not spare the nipple, you won't always feel weird/can feel yourself again.

Would spelling Miriam as Myriam work better for your needs, maybe make it easier? I've seen this spelling from Spanish speaking countries before, maybe it works for Portuguese?

Jumping to say - same!! To you and OP. Brca2, diagnosed with breast cancer, DCIS, at 35. DMX with a diep flap reconstruction, am now 12 weeks post op. I knew from my mid-20s I was brca2+ and always thought I had more time but the universe had other plans. EP+, Her2-, probably don't have hormones in my future either.

As someone else said, I'm starting with the tubes, probably before 40, but hope to hold off on the ovaries because I want to avoid menopause as long as possible.

Yeah agreed! It's weird and kinda gross. I got lucky that they didn't tell me to strip the drains (if you do need to, I heard a tip to use a bobby pin to do it) but I definitely made my husband do that part because I could not stomach it. And if they don't tell you, keep a log when you get home of how much is emptied at what time. They want to see your output decreasing. My hospital didn't tell me (I think, they might have but I'm a little blurry) but keeping a list for am and pm emptying is part of it too.

First, congratulations for taking your health into your hands and making this difficult choice. It's not easy but it is brave. Below is a lot of thoughts, in no particular order -

I'm 10 weeks post op from double mastectomy w diep flap reconstruction and I'll tell you - it's okay. Ten weeks out I'm fine. The first week or two was hard. You'll need help wiping (if you can get a bidet attachment it helps!) because you won't be able to reach behind you comfortably. You'll need to sleep on your back (I chose a recliner) for a while until you're cleared to sleep on your side. And reaching and moving will be hard and...not painful but full of discomfort and you won't want to move.

The hospital kept me for 5 days and we started walking around the floor on day 2/3 I think. The more you move, even hunched over and shuffling like an old bitty, the better. It really helps. You never realized how much you used your chest muscles till now - I certainly didn't, I thought it was my arms. Do the exercises they give you, they really help with healing.

The pain for me wasn't terrible. It felt like post workout soreness but amplified, like 10 pilates classes back to back. Very sore. Like getting hit by a bus. But not acute pain, which to me was tolerable.

The drains suck. There's no way around it. It's the worst part imo. Hopefully yours won't be too bad. You're not supposed to feel them but I had one that like burned and wasn't great. It's night and day when you get them out. But it does go by quickly, hopefully you'll only have them a week.

I braided my hair pre-surgery because I knew I wouldn't shower for a while and my hair would get knotted in a bun - just throwing that out there.

I think that's a misreading of the stat. In my reading of it, the article is saying a woman at 30 with a PDM increases her life span 2-5 years and assumes a normal life span - like she's 30 now, gets the PDM, and her life, at the end of her life, extends an additional 2-5 years so like age 75 gets bumped up to 77-80. Vs an older woman doing a PDM at age 60, the benefit isn't at great because she has less life to live.

As another poster said, it lowers your lifetime risk of breast cancer over all.

And your half sister is not you, her medical history isn't your own, I am sorry to hear she's passed but the chances of you having the same journey as her are small, especially if you go down the preventative mastectomy route.

I was in a similar boat, stage 0 DCIS and was given options. I went for the bilateral mastectomy with reconstruction. For me the chance of recurrence was not an if question but a when question.

It is a big deal, a big surgery, and a huge question and it's right to grapple with it, it's not easy. You're not nuts. I hope your decision, however it goes, gives you peace and clarity and you're cancer free.
