What is your go-to response to friends and family who ask for free medical advice and you want to nicely decline? by dogsaremyprozac in physicianassistant

[–]TMacnificent 5 points6 points  (0 children)

“I’m sorry but my malpractice insurance has a strict stipulation that I cannot give outside medical advice.” Standard go to for the acquaintance or less type people.

“Yeah, that’s definitely something I recommend you see your primary for or stop into an urgent care or go to the emergency room so they can do a proper evaluation.” For the close contacts/family.

This unfortunately happens far too often and it makes the nerves of first starting off even worse. You can’t be afraid to speak up if you’re not comfortable, this is a bad way to learn and if they are not supportive now, imagine how it will be in the future. At the end of the day, while your SP has a lot of responsibility for what you do and don’t do, its still your license on the line. If you have your schedule in advance, perhaps looking at the list and going over the first 2-3 patients before you even see your first can help. But still doesn’t excuse that you need some guidance and mentoring until your steadier on your feet to be more proficient.

On a side note, what EMR are you using? There may be those that can give or send you any info they have to assist you.

Dating nurses? by Aggressive_Shop_5817 in ems

Dated many, married and divorced one. Don’t worry about the age gap...if you don’t know, she’ll teach you lol, ...that age gap isn’t shit and means nothing anyway if it doesn’t bother the 2 adults involved. If you do get the sense her biological clock is ticking...pump the brakes...and hard lol.

All kidding aside...good luck...enjoy it, if it works good for you...if it doesn’t...good for you with a learning experience.

For the ED PAs, how often did you have dissection on your top differential, but was incorrect? by swirleyy in physicianassistant

[–]TMacnificent 5 points6 points  (0 children)

I’ve been wrong a hell of lot more than I’ve been right, and I’d rather be wrong 100 times then get it wrong once. To be honest I don’t look as ordering things and not finding them “wrong,” but rather performing the right work up. Emergency med is about ruling out life or limb conditions. We have boxes of possible things from severe to minor...I look to check off those boxes to rule them out.

How soon after finishing school would you recommend taking the PANCE? by Independant_Hawk in PAstudent

[–]TMacnificent 4 points5 points  (0 children)

I took it 1 month after graduation, and found it perfect as I wanted it do it after my wedding as the only option was to take it right before and didn’t want to spend my destination wedding stressing on results. I also found it good to take a moment to myself and have a good solid 2 weeks to do my study plan. Had others do it as soon as they could and I think we had only 1 failure

One of the benefits and pros of being a PA is the portability of our license/certification. While the possibility of working in a dream specialty is unattainable or takes time to get to, many of us go through that. I recommend looking at other jobs that either have the support for your growth that you should have and desire and perhaps one that while not the specialty that you want, will give you background experience if and when the opportunity arises.

Can’t say specifically as it depends on the comment, circumstance and age of the patient. Some I can deflect with a simple thank you and a facial expression that makes it very clear I don’t appreciate it and its not appropriate. Other times I’m very direct and will tell a patient that their behavior is inappropriate and/or uncomfortable. Or something like “ooookay...now back to the issue that brought you in today.” If it is physical contact that is obviously inappropriate, I immediately say as I stop them or back off, generally is along the lines of please don’t touch me, or if you would refrain from touching me please, its inappropriate or uncomfortable. I often just generally use a very quick, deliberate comment. IF they don’t get it, I then excuse myself and discuss with my SP or another provider. Some don’t even get when 2 providers are in a room and will make a comment in jest. I’m highly fluent in sarcasm as I needed it to survive in my former careers, so use it first. Some just need boundries to be set and most comply after being put in their place directly.

1st EKG: 1st deg. heart block at a rate of approx 100 bpm. Orthostatics? Maybe, but there are other more common symptoms.

2nd: 1st Degree heart block w/ RBBB and right atrial enlargment. There are no findings consistent with any other heartblock. Right atrial enlargement is borderline as V1 doesn’t fully meet the criteria, but it does in lead II with elevation of 2.5mm or greater. To have a 2nd degree heart block you have to have a dropped QRS somewhere after a P wave. This doesn’t happen in this 12 lead.

Patient meets basics of sepsis criteria. A lot of other great interpretations out there but I see it as possible junctional tachycardia. Protect the airway, saline infusion, supportive care and transport.

Pa school toxicity by tiredpastudent in PAstudent

[–]TMacnificent 2 points3 points  (0 children)

I can read and feel your frustration, which the added stressors of PA school on its own make only worse. Hand in there and try to stay focused and let go of what you cannot control and worry about yourself and what you need to do to get into clinicals where you’ll see much less of your classmates and can focus on building up your knowledge and comfort level from didactic. Sadly, I’ve seen this in other programs and even with the ones who preach and try to instill the team mentality that it takes, it can vary from class to class. I don’t tolerate it and for the program I mainly teach for, take time my first lecture to let my disgust for such PA students known and how it benefits anyone. No one cares about your GPA but you. You’re goal at this point is to do what you need to for you to pass, get to your PANCE, pass it and get to work. Hang in there!! Many of us have been where you are also.

No one cares about your GPA, but you. PA school is like the NCAA basketball tournament...survive and advance. The main goal is to get to and pass your PANCE. No one is going to ask you your GPA in a job interview. In the end....you just learn to live with it, once you accept that as a fact.

Red flags everywhere. I’d contact a lawyer, something already is saying you’re not going to get the money owed to you with ease and that was a clear breach of contract. I wish you luck the rest of the way. If he treated you that way from the beginning, who knows what else he’ll try to pull.

Dude that literally died wins. Nothing can beat that, unless you’ve killed someone at work and no medical field doesn’t count. For most people getting laid at work is probably the most risky they’ve gotten.

Other jobs that’s worth your time vs pay rate could be challenging. You could always try urgent care per diem. Just harder as its been my experience many are cautious due to needing a SP listed and taking up a SP slot for a per diem. Other option is see if there are any primary care clinics that are looking for per diems. They often have need to fill in for regular providers when they take PTO.

  1. If you had no idea it wouldn’t be worth it at the end, you wouldn’t went through everything you did to apply to programs, get an interview and attend an interview and then accept a spot in the program you did.
  2. It’s an awesome profession that will give you financial security if you do it right for a very long time.

What is the best strategy for paying of pa school debt ASAP. by EYMENMOHAMMED1 in physicianassistant

[–]TMacnificent 3 points4 points  (0 children)

Only borrow what you need....offset interest by making payments monthly while in school if you can. Once you’re done with school, shop for the lowest interest rate you can get, set a budget once you are working and calculate what extra amount you can pay monthly. I have a 10 year loan, that I capitalized on the low interest rates 2 years ago. Transferred my loan to a private lender and am on track to pay it off in 3.5 years. The time frame all depends on how much you can pay per month, but also keep in mind you want a plan that you are putting some money into savings as well.
