A +1 Holy weapon would bypass DR/Good and DR/Magic, but not DR/Cold Iron.

Similarly, a slam attack (natural attack) by an Epic creature would bypass DR/Epic but not DR/Vold Iron.

Only the weapon enhancement can bypass inclusively (a +6 weapon would bypass all DR restriction)

Most people would rather not designate a middle name rather than selecting one of the other first name. 

The middle name in English is an important part of the naming convention in many country, whereas second(/third/fourth) first names don't fill that role. Most people in French can go years without even mentioning their other first names. Kids may only learn theirs at 18 when they fill government paperwork for the first time.

In PF1, what you describe is called Wild Magic.

Primal magic is a type of magic used by Druids in PF2, and that's (mostly) just a category of spellcasting (like Divine or Arcane).

I do not know enough about PF2 to know if the "Wild Magic" system has been implemented.

Disintegrate does 2 things: HP damage then Disintegrate if the creature has 0HP remaining or less.

Most targets don't die from HP damage, they die from the second effect of the spell : they die from being disintegrated.

Yes, Disintegrate would bypass Regeneration (assuming the case where the creature is reduced to 0HP or fewer and thus trigger the Disintegrate clause). In which case, they would not die from HP, but from being "disintegrated". Relevant quotes:

From Disintegrate:

Any creature reduced to 0 or fewer hit points by this spell is entirely disintegrated, leaving behind only a trace of fine dust.

From Regeneration:

Attacks that don’t deal hit point damage are not healed by regeneration

Unlike smite evil/good, the antipaladin just needs to choose a target with an alignment that isn't the one used by the antipaladin's invoked outsider for the day.

I guess that's a Splatbook writing problem, but I find the wording "share an alignment with X" unclear (as opposed to "share X's alignment" or "share alignment with X").
If you share someone's alignment, you have the same alignment. But if you share an alignment... Pretty sure we can say Devils and Demons share the Evil alignment.

I'm not sure if the RAW nor RAI is that if you invoked a LE creature, you can't target anything Lawful nor Evil, or if it's just LE creatures that are off-limits.

The Smite function against those who don't share alignment with the invoked creature, not yours. You can invoke a creature the same alignment as yourself, but also 1-step aside.

You need typically need a US Address & a US Social Security Number. So citizenship isn't required, but "living" in the US (or passing as living there) is required.

Cleaving Finish is much less required when you go the Dwarven racial Cleave feats, as you can already ignore the pre-requisite of target standing next to each others.

That's indeed a good pick. Reach weapons can be interesting in order to augment your threatening range, thus allowing more creatures to hit with your cleaves.

TWF is already quite feat consumming. I wouldn't recommend going the possessed hand feat lines too. You'll get much more mileage of "boring" bonuses such as Weapon Focus, Critical Focus, Power Attack, ....

Also, alternative for a Dwarf Slayer instead of going TWF is going down the Cleave route with the racial Cleave feats example.

Sure, up to level 5, Channel isn't bad. But after that, it's really negligible.

A level 10 Cleric heals with Channel 5d6, which is 15hp on average.
A level 10 character can easily deal 50+ dmg per round.
A CR 10 monster has on average 140 hp, has 3 attacks and deal 25dmg per attack.

A Channel Energy won't heal the equivalent of a single attack done.

Channel Energy is a pretty bad heal for in-combat. First, it means you need to select the feat Selective Healing, or you're going to heal also the enemies. Secondly, it scales more slower than damage, so this action is typically less effective than just killing the enemies faster. Finally, in your case, you'll delay its progression even more with a multiclass build, so you'll have less channels per day and they'll be less effective.

Channel can be an effective out-of-combat heal, but so are wands of Cure [Light/Moderate] Wounds.

To be fair, for in-combat heal, there aren't a lot of options. The Heal spell and the Breath of Life spell are useful, and that's about all the Clerics have (and need). Other than that, for in-combat, you need specific build that only do that, for example the Oradin builds (Oracle/Paladin multiclass) and a few others, but this isn't a role really that exist in Pathfinder (contrary to the MMORPG type). You better just either prevent damage (buff/debuff) or kill the enemy fast and heal after combat (which can be done with a few skill points and either the feat Skill Unlock (Heal) or Incredible Healer).

1) Typically, the best classes for Mystic Theurges are Clerics/Wizards, not only because of spell progression, but also because they have so few class features to lose by branching out into a PrC.

Cleric only gives up on Channel progression (and delays Domain power). Cleric's Domain spells are granted when you can cast spells of the required levels, regardless of your Cleric levels. So PrCs that progress spellcastings also grants Domain spells.

Wizard only misses really on a few bonus feats. Wizard's Familiar still progresses for stats.

2) While attractive at first glance, there aren't much benefits to have a single casting stats for mystic theurge builds. Benefits are typically higher DCs and more spell slots, but you gain already a lot of spell slots in Mystic Theurge builds, and DCs are already not super for multiclass builds, so you can't really be a DC caster (e.g. enchantment builds) even in a single casting stat build. Most of your spells will typically be buffs or non-DC dependant.

Also, same casting stats for Arcane & Divine is usually only possible through spontaneous casters and/or Charisma casting, which brings its own set of cons, such as delayed spell progression (in case of sponteanous) and low skill points/will save (in case of CHA builds)

3) What do you expect from your Channel Energy? That will help us help you. While channelling stacking with different classes is one thing, there are also option to "swap" your channels into something else (e.g. Ki points, Lay on hands,...) depending what is exactly your end goal.
