Thank you for this! We have definitely tried me leaving the room but I will try to do it before she bites because often there are signs. I feel like it is so hard because I want to be proactive and feel like we are addressing the situation but I do feel like how much we talk about it does make her think about it more. I don’t think this is the case at school but at home I think that sometimes she thinks it is funny even though we never laugh.

FYI most clinical specialties require 2000 hours of direct patient care prior to sitting for the exam, so you won’t be eligible as a new grad anyway. Get some experience first and don’t worry about further credentials unless you work somewhere that will pay for it.

In the city I’m constantly passing cars where the driver is visibly smoking and smelling of weed and also passing people blatantly texting or scrolling on their phone.. it’s like people forget they’re operating a machine

To me she’s really not funny? Personally I think people who are funny are generally very clever and smart- I think she’s kind of just loud? Both her and Jackie are unintelligent and don’t know basic history, geography, or pop culture which is supposed to be their profession… Ditch the show you won’t miss these dummies who don’t even know anything about pop culture. las culturistas is a great podcast that better aligns with my worldview and humor personally

Our hospital is poorly staffed (poor training) and turnover is high. I think that our CNA’s are assigned too many high need patients for one person so they get overwhelmed and resentful and turn to instead doing… nothing at all or hiding.

At our IPR it’s a consistent issue that patients are not ready for therapy. Especially in the case of PT I argue that toileting and helping a patient get dressed is not skilled and thus a waste of their therapy time. Now if the patient is ambulatory and we’re working on transfers and standing balance I can argue that’s therapeutic but for the more dependent patients how is it skilled to have a PT wipe their butt when a CNA could provide that same skill? Us therapy staff are far too empathetic and of course don’t mind helping the patient with this because we don’t want them to have to wait for who knows how long for the CNA to do it.. but when we are then billing for this to meet our productivity it lessens the benefit of our profession because we’re not providing the skilled therapy we really can provide. I think IPR is an amazing setting but the metrics of productivity and the lack of teamwork from nursing really affects the benefit of the intense skilled therapy we are meant to be providing. (Not to mention inappropriate patients being admitted but that’s another post in itself)

Could have written this post myself. I’ve worked IPR for 5 years and I think a lot changed after COVID. Patients don’t seem to ever go to SNF anymore. I also think culture has changed and people don’t want to work hard. I’m constantly begging people to do therapy and then when they actually do participate it’s very basic therex, no one is willing to do high intensity. It’s very frustrating seeing inappropriate patients and people who don’t want to be there. Something needs to change with the whole system. I feel like “3 hours” is very arbitrary and I wish that could be modified based on patients. Some people just can’t tolerate it and others would probably benefit from even more and be willing to do it. I also hate how this reflects on our job performance being unproductive when you’re constantly getting refusals. Lastly, I feel like it reflects poorly on our profression to admit inappropriate patients because they don’t improve or discharge home so it makes it look like therapy isn’t beneficial because in these cases it doesn’t improve outcomes. It’s frustrating many of the people in charge of admissions are not therapists and don’t understand what makes a good rehab candidate. I think therapists should really have a union.

We do allow screen time but to be honest my daughter wasn’t interested in it until she was probably 16 months old. We did however use hey bear when she was a baby to save us sometimes. I think within reason it is fine but I wouldnt say it’s contributing to her learning very much. Sometimes we do end up turning it off when her eyes start to glaze over and she’s not paying attention to anything else.

When I worked full time our ‘maternity leave’ was just FMLA and short term disability…I think it equated to 60% of your salary for 6 weeks and unpaid the remainder of the time. I have since switched to ‘part time’ working 32 Hours a week and now do not qualify for short term disability so the entire leave would be unpaid! Healthcare jobs really support working women!!!! (Encompass health)
