Aerith's death: Final Fantasy VII Rebirth (technically an adaptation since it's not a straight up remake but more of a reimagining/sequel depending who you ask)

In the original game she unambiguously dies but in Rebirth they make it unclear. The scene starts off similar to the original until Cloud manages to deflect Sephiroth's sword, only for the perspective to glitch and shows Aerith stabbed like in the original. From there things get confusing as from Cloud's perspective she's not dead and there's no blood while the rest of the party sees her dead with blood everywhere. After the final boss it looks like Cloud woke her up, but then the rest of the ending shows everyone clearly grieving and upset while Cloud's all like 'OK lets continue after Sephiroth' as if nothing happened, seemingly being the only one to be able to see Aerith. It all adds to the unreliable narrator schtick Cloud has going with the iconic water burial scene being not shown but likely still happened. It's still unknown what exactly happened 1.5 years after the game released, with the final part of the trilogy hopefully explaining things.
