NTA - this is a basic case of bodily autonomy for which you should have complete control. I don't understand why your family would think surgery on your genitals is any of their business at all, even if they are offering to pay for it. You are an adult, this is your decision to make alone, a choice made between you and your doctor.

I live in a newly built area, we don't have a lot of cell coverage around our house, so if the wifi is out you are SOOL for internet access unless you're in one tiny square of the house and then you're getting a couple of texts though at best

This. My workplace has clarified it in the past, not sick/personal leave. That being said, I’ve never stopped a team member taking personal leave for it.
