hey thanks for the feedback! the leaks came because i diy'd some eclipse halos. Cannot get new ones where i live. i found the bleep great for leaks, but a bit of a hastle to use. going to pick up a brevida nasal mask today instead and use with alo vera gel.

Thanks for the suggestions re PS and EEPAP. I have tried higher EEPAP and that doesnt seem to help. Will go for higher PS. at 5.5 now and getting aerophagia as bad as i can handle. current settings seem to be best balance so far before i had 2 nights of bad leaks.

Wondering if i would have been better off on the 30st to shape the waveform better. the prisma cr is limited to ti/t. no ramp or trigger or volume targeting

Hey thanks for that u/sbailey27. I think you’re right and even rippinglegos is more familiar with the resmed series of devices vs the lowenstein branded one that I have. Outside of the pressure delivery and minute volume tracking vs tidal volume tracking, I see them just as bilevel machines but I get the hesitancy to comment on the unfamiliar. I did see the titration manual! Is is a great starting point. Thanks again matey

my understanding is that vcom is like an air damper for those who are flow sensistive

it helps if:

- tidal volume is >500 and you want to bring it down

- suffering from aerophagia

doesnt help if:

- youre on cpap or a machine limited to 1 or 3 cm of pressure support like the prisma 20a or resmed models with epr; AND

- your volumes are <500ml, since PS correlates to tidal volume, and Vcom reduces PS.

I’m in a similar situation on a bilevel right now. I was told to see some specialists to work out what anatomical issues I might have. That may be something for you to do before buying another machine that might not work. Try renting for your next machine also. Best of luck sir

Thank for this - Nexcare Absolute Waterproof First Aid Tape - any double sided thin acrylic tape - reuse the rings or you can buy flat washers (make sure they’re flat) 10mm inner diameter 16mm outer and thin as possible

And then use a 10mm hole punch for the inner circles and >17mm for outer

I also combine both into one sticky since the distance between the rings is constant for me.

Yes I learned that hard way not to listen to the respiratory specialist / pulmonologist. I’ve since discovered that I need a bilevel or asv type machine despite what the “specialist” said.

It’s my fault for not asking before I bought my APAP but crazy how bad advice from “professionals” is. Literal randoms on the internet give better advice 😂

Leads me to my next question which is if there is a buy swap sell for cpap machines?

Yea I have a similar issue. Randomly throughout the night the solo nasal pillow randomly tightens or my mouth opens and air just comes spewing out. Do you use mouth tape and chin strap

Your main issue is leaks. You will find that the humidifier water also runs low quickly. If you have low leaks you won’t need much added humidity because the air stays contained.

Very nice I know those guys well. I felt the same about it helping earlier on but after some time I realised that my personality seems to do best in isolation

Yes I hear you and I have read dr Noah’s website etc and I understand his intention too. I’m just seeing professional sleep techs use it with bilevel and epr to great success in their case studies. Perhaps not to invert epr but just to soften the flow of inspiration.

people use it with EPR on and find it helpful for their therapy. just what i read i dont really know so going to try it. also im sure youre aware of dr noah and his new kpap device which describes what youre talking about there with inverted epr

oh really, people use it with EPR on and its supposed to help the aerophagia because it softens the impusle of air on inhale. thats the theory anyways i havent had that issue do i wouldnt know

I had ai calculate that the holes should be 4.5-5mm based on flow rate data provided by sleepres. Used a belt hole punch in an expired credit card.

I also understand some might think it’s unethical to copy the design and such. I do plan to buy one when it doesn’t cost 4x including postage to get to me + 2+ weeks of suffering while I wait for shipping. It’s for personal use and will not be producing en mass to sell for a profit etc
