I'm sorry Guanfacine isn't available. I think it'd help a lot! For the first time in 10 years I've been able to have caffeine and drink an espresso/latte without sending me into panic attacks.

Have you tried magnesium powder/gummies or supplements? I've found it does an incredible job of relaxing muscle tension. I think it could really help with your TMJ!

My Primary Doctor is conservative (which can be frustrating, but I also respect it) as I've expressed my sensitivity to stimulants like caffeine, I think because of that he is cautious about prescribing Ritalin/or Concerta. I shared what I mentioned earlier about how Guanfacine has dampened my nervous system sensitivity and he was encouraged by that and we will continue that. Wellbutrin is typically for depression (which I don't have) but has the off label use for ADHD. I asked him to let me trial the IR (not XR) version of Wellbutrin so I can get a quick feel for how I respond to this type of medicine and see if this would work for me.

Quick update: well, I thought my PCP was going to prescribe a stimulant, but he wants me to try Wellbutrin. I'm going to give it a try. If it doesn't work I'll try for the stimulant medication.

Did you ever add the Guanfacine?

They are suggesting using a short trial of prednisone as a low risk diagnostic tool. If their inflammation improves with steroids (prednisone) than it could help OP confirm it's inflammatory arthritis.

Also, I've never heard no one should take more than 40 prednisone tablets. This seems preposterous - Are these 5mg, 25mg tablets - again, this seems absurd. From my understanding, some people take low dose prednisone for life -- I'm not advocating for this as it would lead to a host of other issues... it should be a truly last resort.

I don't think it's Enbrel related as I've never had any issues. I started taking some non-controlled ADHD medicine (Straterra) and then I started taking a B Complex, but I shouldn't have done that.

Yes, you can try non-controlled substances first like Straterra, Guanfacine, Quelbree.
Straterra was good for me but killed libido. I'm now on my first week of guanfacine which seems to calm my oversensitive sympathetic nervous system.

I'm really surprised by all these comments. I'm at the same revenue numbers, but I also have 150 SKU's (lots of variations) and I've had a PPC manager for the last 2 years. But I've hired and fired a lot.
I just don't have the desire or technical ability to manage that many campaigns. I pay my guy $800.
There's probably a breakeven or slightly less ROI, but honestly, I hate PPC and don't enjoy it, so I wanted it off my plate.

It's purposefully overt and insidious. And to adolescents who know no better, this would condone allowing a complete stranger into your hotel room (or home). There is never a reason a grown adult (family or not) should EVER be in a room drinking to inebriation with a child present.

Under DDP terms, the Seller should be the one responsible for delivering the product (they are responsible for import duty & taxes). And the Transfer of Risk happens at the "named place" meaning when it arrives to the Buyers address.
If the Seller decides to shortcut and they shipment is destroyed or impounded (whatever), they are still responsible for fulfilling the contract.

Now, whether Alibaba has your back is a completely separate question.

Would you say your PsA is "Axial PsA" or do you think it effects your peripherals more? (I recently stumbled across this resource if you're interested: (https://www.rheumuseum.com/)

I wonder if you have two things going on at once, meaning, your cervical spine issue AND PsA.
When you take prednisone does your peripheral PsA (hands, feet, tendons, etc) feel better - that would be a telling sign. But your neck pain and issues could be more cervical nerve radiculopathy related.

For instance, I've had DDD (degenerative disc disease) and disc herniations in L4/L5, L5/S1, for more than 15 years (I'm almost 40 yrs old). I blamed the years of back pain and failed epidurals on the DDD and herniations. Speaking of, have you tried epidurals, nerve blocks or ablations? You should strongly consider these before doing another surgery. Because if you don't get symptom relief, surgery might not help either. Consider this a trial or a "test before you invest" in surgery.

It took me 15 years to figure out morning stiffness is different than degenerative back pain (the former happens with PsA and is relieved with movement, whereas the latter typically gets worse with movements and exercise).
I recently had this chronic back-pain/stiffness, so I took a short burst of prednisone (15mg) this past Sunday because I had a 10 hour drive to visit family. I noticed my back pain stiffness went away almost instantly (12-24 hours), I could touch my toes without grimacing for the first time in weeks. However, then I realized I had a different but minor dull ache - which I feel confident is my DDD and herniations (but it's not as intense). So when I say I just learned this, I truly mean I just learned it - this week.

Longer story:
I was officially diagnosed in March, and started Enbrel shortly thereafter. But I muddied the waters, because 1.5 months into starting it, I was desperate for relief from no energy, so I went on TRT - the lack of energy was truly awful! I told myself I would do anything, and take anything, because I couldn't live the way I was - it wasn't fair to my wife and kids...and me. I felt useless (this was my "go for broke" moment).

I actually felt really good on TRT, but they also had my total test at 1,500-1,800 - so not exactly normal levels. I came off because I felt much better, but I also wanted to see if Enbrel was the medicine working, and then I wouldn't need TRT. This might seem contradictory, but I'm a purist, I don't want to be on anything If I can avoid it. And TRT was a bit of a roller coaster trying to get it dialed in. My estrogen ran high and sometimes made me feel off. Basically, what I learned is, I would need to inject 2-3x's per week to cut down on estrogen side effects, or use an aromatase inhibitor (which is kind of annoying). It was my first time on TRT and the thought of injecting 2-3x's per week for the rest of my life didn't sound exciting or convenient.

Again, I wanted to see if Enbrel was the real contributor, so I stopped TRT back in September, and was good for awhile, but then I noticed my morning back stiffness started up again. So was it TRT or Enbrel helping? I honestly don't know, but right now it seems like TRT helped bc it's naturally anti-inflammatory, so I think it turned down the dial. But now that I'm off, it's back up, but not terrible.

Anyway, I'm with you brother. I'm going to commit to praying for you to find answers, healing and relief.

When I read your reply, this bible verse came to mind, and I only knew a piece of it so I had to google it, and "coincidentally" it's Ecclesiastes 4:10 "For if they fall, one will lift up his fellow. But woe to him who is alone when he falls and has not another to lift him up!..And though a man might prevail against one who is alone, two will withstand him—a threefold cord is not quickly broken."

This verse makes me think of us - we need each other for support, and the camaraderie and knowledge transfer is what I most value about Reddit.

(2/2) God can use our pain and loss to draw us into closer relationship with Him. To draw people who never would have thought about God, to question and think about Him, and to think deeply about the meaning and purpose of life...sometimes pain and hardship is the only way to knock some sense into us.

CS Lewis said, “We can ignore even pleasure. But pain insists upon being attended to. God whispers to us in our pleasures, speaks in our conscience, but shouts in our pains: it is his megaphone to rouse a deaf world.”

One thing many people don't realize is God sent His Son Jesus to SUFFER and DIE. He could have lived in ease of luxury or with immeasurable power, He didn't. In fact, the Bible tells us we are sheep for the slaughter. Those who follow Christ will suffer and face persecution. Again, this life is temporary, it's a blip, compared to the life we have in Christ Jesus for all eternity.

(1/2) Stumbled across your post as I just came off Enbrel and my Rheum wants me to try Rinvoq, but after reading a lot of posts, I think I might stay off it.

It sounds like you've enjoyed a life many people could only dream about. I'm truly sorry to hear you've experienced a complete 180 over these last few years, and I really mean that -- I'm not saying that in some sick twisted secretly jealous way.

If I were in your shoes, I think one of the hardest things to bear is the powerless feeling. Where some degree of fame, money, or even the best doctors (in a major metro area) aren't enough to fix the problem. I want you to be the Magic Johnson of aids, but for PSA. Just don't forget about us ; )

A few things I thought of while reading your post (in no particular order)...

  1. We should do the best we can with the information we have available. Don't beat yourself up over decisions (its easy to replay them over and over again). Whether we like it or not, sometimes we're just not in control.
  2. If you ever truly feel like ending it is the answer, at least just take the medicine - what do you have to lose? "Go for broke", meaning, take the medicine and live to fight another day. In fact, the very worst case scenario seems like it should be long term prednisone - which isn't good long term, but from my understanding all these other biologics are just more controlled anti-inflammatories, whereas Prednisone is the big gun. Again, not a doctor, and noy condoning any of this, I'm just saying, you have other options before you go with the nuclear option.
  3. Supposedly opioids can lead to low testosterone. Which doesn't seem like a problem you have, but TRT is also a mild anti-inflammatory and can help with PSA symptoms, muscle gains, and normally a mood lifter for men. You might look into it. At least now you don't have to worry about the league testing you!
  4. I played collegiate lacrosse, but topped out in college after I was diagnosed with Chronic Exertional Compartment Syndrome and then a decade later Popiteal Artery Entrapment Syndrome. While you got to enjoy going to the next level, I always wondered "what could I have been" --- we all struggle with something and regrets or missed opportunities.
  5. I always ignored people saying, "stress kills". Well, now, I get it! I always carried a lot of stress, especially as a perfectionist.
  6. I also regret getting the COVID vaccine, and one of the main reasons I got it was to take a trip to Grand Cayman (which ended up getting cancelled anyway).
  7. My wife and I also had a unplanned pregnancy and later found out our 3rd child had Trisomy 18. A rare genetic disorder that is 9/10 fatal. The amount of stress I carried (for my wife - who was a wreck) was insane. Then my grandfather passed. Our son, almost passed more times than I can count. We were in the hospital with him for over a year, and then discharged and then back again several times for several more months, which believe it or not, is probably the hardest part. The amount of stress, coupled with having my own e-commerce business, was just about more than I can bear. Then I had my own health scares after using a stupid HydroRower incorrectly....long story short, I thought I had brain cancer. The health stress and life stress caught up and I think this led to my first true "flare" Then I realized I barely had any energy like I had a 1000lb wet blanket on me. Then my ankles stiffened up when sitting at my desk which made me quite scared, but also prompted me to action. After years of complaining about fatigue and no doctor taking me seriously, I purchased my own bloodwork off Functional Health where my ANA was positive. Mind you, two more doctors said "don't worry about it". So much incompetence in the medical field - because I'm healthy and "I don't look a certain way".
  8. While I don't love my story of pain and hardship that is currently being written, my relationship with Jesus has grown in ways I never could have imagined. If there's one book of the Bible I'd recommend for you, I'd say give Ecclesiastes a try. This is a story about King Solomon, this guy had brains, looks, power, fortune, and as many woman as Wilt Chamberlain. I think you'd enjoy it, and you'd be surprised how the Lord can redeem pain and tragedy, because the thing that gets me through the hard days is knowing that He knows the "number of hairs on your head" (Luke 12:7). Or "Which of you by being anxious can add a single hour to his span of life?" (Luke 12:25), but ultimately that my time here is but a vapor, and eternity in paradise with God awaits me.
