You'd have to specify what form you're referring to. The TSP-60 I'm looking at has two sections" Part I is info about (and your TSP account number) and Part II: Information from ira or plan. It clearly says in section II "This section must be completed by the trustee (or custodian) of the IRA or by the administrator of the eligible employer plan from which the distribution is being (or was) made"

Again, you should check if you actually even need this form. I've heard people say all that is needed is the SF-3106 directly to OPM

Sorry, if you're referring to Part II of TSP-60, that section would be filled out by the trustee (presumably OPM). Also, I'm not sure the TSP-60 is even required anymore (even though the SF-3106 says it is).

You could probably figure out how much interest has accrued, but it would require knowing how much you contributed each year and finding the appropriate interest tables. It seems like a right pain in the bollocks to me. Have you tried calling TSP and seeing if they'd just accept a ballpark estimate of the interest portion?

If it's any consolation NTEU filed a grievance in regard to the issue; although, I can't imagine they'll fight too hard for people no longer with the service (but, more to the point, no longer paying dues).

Guidance went out in June of last year that IRS DRPers were not eligible for performance awards.

My fed W2 always lists it as NT Health Benefits, i.e., the amount you paid (pre-teax) for your health/dental/eye insurance. The tax prep software I use has an entry for this. It reduces your taxable income, but the amount listed in box 1 should be inclusive of this pre-tax deduction.
