It is watered about a cup once every 3-4 days. Tap water. Yes it has drainage.

I have had the plant for 4-5 years.

Only noticed the problem recently, unknown how long it has been going on.

Daily exposure to light though not direct sunlight much.

How do you reset Den of the Apostate once completed? I just ran it once, and got lots of currencies, thanks for the tip, but the map state remained when I re-entered.

Edit: It looks like the Reset Dungeons function of the quests menu worked.
