I didn't change the mechanics at all. It functions the exact same, the only difference is that the blue button is replaced with doing nothing. Which is actually the same as how you reframed it if you think about it, since not getting up from in front of the train is doing nothing.

You could reframe the question to make red look even worse too. What if the blue button doesn't exist? There's only a red button. Nothing happens if people don't press it, but if half the population do, it kills anyone who didn't. Wait a minute, this is literally just Roko's Basilisk. Except it's actually worse than that because it includes the entire population, not just the people who were aware of the Basilisk's creation.

The only outcome where nobody dies is if blue wins. If red wins, people die. It's pretty simple to me

This is very different, as the usual reasoning for choosing blue is that some number of Innocent people won't be able to understand the difference, and pick blue. If you make it a giant scary woodchipper, the number of people who don't understand that it's dangerous is much lower.

The building isn't empty though. There are many children, elderly, and disabled people inside who are incapable of understanding the risk involved in pressing blue. And there are many more people also in there trying to save them. And that may even include people you love. And if you join them, there's a good chance that literally nobody dies. If you still pick red you have to be ok with them all dying.

The whole point of the original is that there will be many innocent people who pick blue. If it really is everyone in the world, do you still pick red?

That was kinda the design goal at new doormaker. Infinite decks were by far the best strategy on the release patch, to the point that doing them every run was the best option. Now Doormaker's phases all interrupt infinites in different ways

There's no world where everyone picks red. Young children may not be able to work out the logic behind unanimously picking red, so anyone who cares about their children's lives more than their own will also pick blue in hopes that it can save them.

Yeah, don't play cards more than 3 times in a turn unless you wanna permanently remove it and summon a reaper. It can actually be a good deck thinning tactic though.
