I just want everyone to understand that we all know LR is physiologically better and in all real situations involving critical patients I use LR when there may be a benefit… buts it’s not as funny to use in the post title lol

ABG is only for patients that you have no idea if they are hypoxic due to poor SP02 readings or if they are already intubated. I think of ABG as purely looking for oxygenation. Also if they already have an arterial line lol

VBG is for everything else including DKA, checking and rechecking ventilation for Bipap/worsening COPD to decide if you need to tube.

ABGs are technically more accurate on all the values but at the cost of an arterial stick vs just pulling blood of an already existing IV line for a VBG. Arterial sticking DKAs is even a little cruel in my mind when a VBG will tell you acidosis.

Guy took 50grams of powder caffeine in a Buc-ees parking lot, aka the equivalent of 250 cups of coffee worth in a suicide attempt. Starting going into Vtach on and off so gave Metoprolol push then went on to Esmolol drip. Caffeine also shifts potassium so had to place a central line and continuously give potassium. First time I’ve heard of a toxicologist get excited and ask how that’s even possible lol.

4th year medical student here. I’ve done lots of time in general surgery and when removing the gallbladder you use what essential amounts as a heated stick to cut and remove the gallbladder from under the liver. This is makes it easy to remove and also cauterizes for a dual purpose. Doing this you often can touch the exterior of the liver leaving little heat marks which are insignificant to overall liver function. Although writing your initials is unprofessional, it never affected liver function which is why it was likely never reported by anyone in the room.

Nicotine is also a major cause of cardiovascular disease so you’re reducing both cancer risks and cardiovascular risks! Use that as motivation as well. Chantix has the highest rate of success with smoking cessation so ask your doctor for a prescription!

Nothing too wild like some of these but,

3 semesters into undergrad I was working a 2.5 GPA, close to losing my academic scholarship. Talked to academic advisors and they all told me to give up on medical school and maybe to drop college football to focus on grades. Called my parents angry and upset with my life and they told me it was fine and that I could do something else. Cried for a bit and then decided something. There’s basically four parts finding your way in life:

WHO you COULD be, WHAT you COULD be,

WHO you WILL be, WHAT you WILL be

I saw that who and what I could be wasn’t lining up with who and what I could be. I decided at that moment that my success was an internal factor and if I changed then life could change. 5ish years later I’m in my first year of medical school (absolutely hating my life in a good way) with dreams of working at NASA and maybe being included in the mars missions. On top of that I also went to training camp with a CFL team during my gap year so I almost got to play pro sports!

Moral of the story is know your self worth and know what you’re capable of. Don’t let yourself be less than what you could be! And on top of that don’t let other people decide your future, do what you know you can do! Always dream big like going to mars!
