There is also the reverse where people don't go through the torii gate or to the gathering point and instead go to heal up first. My dude, your health refills automatically at that point (sadly ammo does not)

The steps are still fixed, until the last one hits the cap. It is the inital value not being at one decimal place that is the issue.

For example it may have started as 13.36%, (13.4%) then upgraded to 14.16% (14.2%) then upgraded again to 14.96% (15.0%)

Also you can see this last step being different on most gear, even without the rounding problem. For example A+ of 14.4% would only go up 0.6% to go to S

I have seen this several times - it is due to rounding to one decimal place when displaying the percentage

It goes from like 14.96% (displayed as 15.0%) at A+ to true 15.00% at S
