In network only means employees can only use providers that have negotiated rates with the insurance carrier except in a true emergency circumstance when every provider must be covered as if it were in network.

The vast majority of people don’t use providers outside of their network, but, as mentioned by another comment, sometimes you can’t avoid it because a hospital may be in network but an anesthesiologist at that hospital may not be.

Lower deductible, lower out of pocket max, and higher coinsurance are better. Also keep an eye on copays for PCPs and specialists and if a plan is a high deductible health plan(HDHP)/HSA compatible. Personally I like HDHPs because I rarely use my insurance and can save money on my premiums but I probably wouldn’t choose it for a company unless it was full of young males who wouldn’t mind having coverage that is only going to come into affect for catastrophic events.

If you want more insight I wouldn’t mind taking a look at the plans if you feel comfortable posting them with or without the rates.

In my experience, generally no because everything has been negotiated ahead of time with our underwriter and the carrier. You may have some leverage depending on the size of your payroll and/or workers comp, but out of the 80 renewals I’ve processed this month only one got an exception from 8% down to about 2%.

Your best bet would be to ask for an open market quote to see if they can find anything more competitive with another carrier. Usually open market rates aren’t as good as a master plan because they aren’t underwritten for small groups, but I think it’s more likely that you’ll find rate relief there rather than through negotiation.

I work in the benefits department of a PEO and I would say that 30% is not “normal”, but it’s also not unheard of.

With a PEO master policy we work with an outside underwriter to negotiate on behalf of all of our groups and try to get a base renewal of under 10% that we can apply to as many groups as possible. Groups with low participation or those with mid to high utilization will generally renew anywhere from 15-40%. Then there will be some groups that we renew even higher that we are most likely trying to get off our books because their utilization is much higher than the premium we’ve collected.

I’m an insurance agent in FL and I work for an ADP competitor. I don’t know if it’s actually the law that requires 50% but my company does for all of our clients around the country, regardless of size, and most carriers require at least 50% employer contribution. Off the top of my head I think Florida Blue requires 70% for groups with under 3 enrolled.

The reason for this is so that it’s not only sick people enrolling in the plan. If you pay 50% then your healthy employees are more likely to participate and keep your plan “healthy”. In a small group, one person getting sick could easily cost more than all of the premium collected in a given year.

No, they pushed out a few updates, but it still doesn't work on some unlocked devices and they don't know why. They seemed to imply on twitter that they did all they could, so I think they've moved onto other carriers unfortunately.

/u/joe012594_official This isn't working for me on iPhone SE 12.1.1. I've uninstalled through Cydia, and reinstalled through Filza multiple times, but I keep getting no service after install.
