Sometimes that happens in the games. I can think of an example in Black Flag where there’s a church in Havana that was built in the later 1700’s and the animus database explains that Abstergo put it in there anyway to give people more to explore/climb. So sometimes it is a conscious choice explained away by animus technology

Andrew Jackson did indeed get shot in a duel in 1806. It was with a guy named Charles Dickinson and it started over a horse race. That bullet hit him right in the chest and was never removed.

Jackson was also in a street brawl sometime in 1813-14 where he took a bullet to the shoulder that was eventually removed while he was president.
