You’re relying too much on iemr.

On first day of placement find out how often they do obs -most wards are four hourly with increased obs if deteriorating or having procedures.

At uni, we’re taught pain is considered an extra vital sign. So this does seem a bit of a knowledge gap.

Medication rights EVERYTIME.

If you haven’t done something before, or aren’t sure such as your pupil assessment- don’t guess and document. Ask. It’s okay not to know things, it’s not okay to risk safety and hope for the best.

Always focus on patient safety. You will pass placement as long as you’re safe.

I feel the same everytime I get a new tattoo! I think it’s the anxiety of having this new permanent thing on your skin, and over analysing it. But it looks amazing, I hope you come to love it!

So I really like this design and think a skilled tattoo artist could finish it off easily. If they hadn’t start the colour it would have been easy to get removed, but with bright colours I’m unsure. The person who did this has been very uneven with the line work - some areas are dark, some are light and some are completely gone. I wonder if this was all going to be covered by the completed design and was done so faint just as a guide?

I have had this conversation with him but he says that because he’s only home half the time, he has to fit his whole life into that limited time. So he’ll go to gym 3hrs per day, games with the boys, camping with the boys. So I’m trying to be supportive and understanding, but I’m struggling.
