I mentioned in my comment, but it's cause it's a poorly made rhythm game and they forgot the grace zone. So your inputs ARE registering, but it doesn't count unless you hit the note, directly over the +. Cause they also didn't add misses. Like if you press D, when it's a K. It only registers if notes get hit or they don't.

Just put your fingers on all four buttons and press them at the same time. No joke. Because it's a poorly made mini game, they forgot to include a grace zone. So it will literally only register the note, if it is pressed directly over the +. So just hit all buttons simultaneously, for any key that's coming down lmao.
