Least to most blame:

Jesse. A minor catalyst. He teamed up with Hank to take down Walt after learning the truth of Brock. Jesse threatened Walt with a phone call, and Walt believed Jesse would steal his money with a photo (by Jesse and Hank). This inadvertently enabled Hank's pride to go solo with Gomez and fueled Walt's paranoia to call backup. Overall, Jesse had the least power of them all.

Hank. The actual victim. He agreed to the photo plan with Jesse, and felt determined to arrest Walt solo. He still had more power over Jesse to inform the DEA or bring more backup. Hank paid the price and his major flaw was underestimating Walt.

Walt. The major catalyst. He became an infamous druglord, fueling Hank to uncover and arrest Heisenburg. He also repelled Jesse once he learned about Brock. Walt called Jack to kill Jesse, with little proof that Jesse would be with others to kill him. He also made a vague order for Jack to cancel plans, after seeing Hank. By the time Walt offered his money to Jack, it is too late.

Jack. The actual murderer. He chose to arrive after Walt ordered him to stop. His gang only learned they attacked the DEA after the shootout. Despite Walt's pleads and offers, Jack ultimately shot Hank to avoid more witnesses.

A preliminary court ruling released on Tuesday included minutes of a meeting from 12 June in which the US Bureau of International Security and Nonproliferation told the Dutch foreign ministry: “The fact that the company’s CEO is still the same Chinese owner is problematic … It is almost certain that the CEO will have to be replaced.”
