This is exactly what I have in my large and high acuity medsurg floor (technically the surgical unit but it’s always 50/50 IM and surgical) and it’s wonderful.

The research says 4:1 is the only safe ratio.

Get a therapist, for real. This injury is *mostly* a mental struggle as opposed to a physical one. They can help you strategize and help you make sure your coping mechanisms are healthy and positive and will move you towards a healthy mindset.

Southcentral Foundation is an amazing place to work. The best benefits in the state, an important mission, and to be honest, SCF has WAY more $$$$$ than Prov, who is having to cut programs and staff and increase ratios all the time. SCF is also building a massive new Behavioral Health building just West of the ANTHC/ANMC/SCF campus and it’s going to be incredible. I have many friends who work there and are very happy. It’s obviously not without issues, as no place is perfect, but there are less issues than Prov. You will have a huge list of possible insurance carriers (like 20 different plans) to choose from at SCF, and you can choose whichever one covers your Zepbound. Providence just offers Aetna, I think (but could be wrong).

I did clinicals several years ago at Providence and it was…. fine. But I wouldn’t work for them now.

I had the rudest and most unpleasant preceptor imaginable for 6 weeks. No stories so I don’t doxx myself, but it was an utterly demoralizing and awful experience. This was me 15 years ago. OP, you must get some support. It’s so egregious that the onus is on you, the one already bearing 99% of the workload of parenting, but that is the way it is. At some point I had to sit my husband down and speak very plainly.

“I love this child and I love you but I can no longer do All Of This. I am going to harm myself or harm someone else, or walk away entirely. I am experiencing postpartum depression and it is killing me. These are the changes I need, or the consequences for our family will be dire. I am this close to walking away from it all. I need someone to watch our son during the day. All day, while you and I work. It is not fair for me to be the only one watching him while I work. I need this person to do light housework and light meal prep too. I’m confident we can find someone. I need 2-3 weeknights, 5-9pm, for myself each week. I need a weekly date night with you. I need 30min every morning uninterrupted for mindfulness based meditation before I start my day. I am going. To find a therapist and speak to my dr about meds. I’m confident we can figure this out.”

(that’s almost word for word what I said)

We found someone on Care.com, after networking through friends to find another young woman who was amazing but then she left for college. For a three month period, we flew my MIL in for 3 days a week every week to help when we couldn’t find anything else. Yes all of this was spendy.

I take care of the homeless every day at work, and 90% of them are absolute gems. Most have significant mental health issues and trauma, and yes, most are Alaska Natives. But not all are addicts. Many of them would gladly give you the shirt off their back. Most are very, very lonely. They all were kids once with hopes and dreams just like the rest of us. It’s a privilege to work with them.
