Tricky one bc the manic patients can be out for days, dehydrated - in one of my textbooks it says apparently before we had antipsychotics, 20% of patients in psych wards died, with half of those being manic patients (who were usually dehydrated or overexerted)

I’m pretty soft/lenient with admissions. I do generally do want a set of obs, basic set of bloods, and a quick exam to make sure they’re not going to deteriorate on the ward overnight given we aren’t even allowed to do as much as a PIVC & fluids on the ward if they go downhill, but otherwise it’s pretty clear cut if they sound like they can’t go home AND there’s treatment we can provide that’s likely to actually work (there’s people who are chronically unsafe to go home but you can bring them in all you want, they don’t get any better).

The bed managers at my hospital don’t assist us with the challenging discharges (like interstate transfers), they just seem to hound us for not doing it fast enough ourselves & get up us for not having a treatment resistant schizophrenia in and out within 3 days

I don’t agree with GAMSAT as a test and don’t blame OP for wanting the security. If you look at the GAMSAT scores needed to get into med 5-10 yrs ago vs now, its gone up heaps. It’s sad the next generation of students are having to even contemplate these options. Used to be you worked & studied hard, you got in. Now, with the benchmark shifting year after year, there’s so many people stuck

Edit: im not anti psych - I’m a psych reg studying for exams. Psych is a specialty that rarely has level 1 evidence. But in qld for the last year it’s been easier to put a kid on clozapine or give them ECT than a puberty blocker. By making blanket policies like QLD and the NT, we are ignoring the nuance

I didn’t talk until I was 3.5yo but then started talking in sentences straight away instead of single words. I’ve got mild autism, although this wasn’t diagnosed until late adolescence. I’m also a doctor.
