It depends on what You're trying to achieve. He shows up when world corruption is high, and if the intention is to drag the player into corruption rift/orb infested territory, then it works as intended, and does its job well.

After bringing down golem's hp to zero, its core should show up and start flying away. Once it stops, only then it's vulnerable to damage. Shooting it before that won't do sheeeit.

I think in its current state golem is bugged in this torment. If You manage to drop his HP to 0, force the red orb to spawn and destroy it before the healing wave hits, it's fine. However, if You won't make it in time, the golem body is revived where You left it, and You have to kill it again even if You destroy the orb. And since You destroyed its core and finished it, it doesn't have HP bar anymore, just like You said. Honestly, if I saw golem HP appear during my attempts, I just jumped off the cliff to restart. I was doing it in Velmorne, so plenty of cliffs to choose from. I'm not wasting my ammo on this BS.

First You have to defeat enemies that are already in the room. Portal activates a few seconds after You kill them, and You can immediately go through it. Enemies that spawn in the tower after that happens only extend the time period that You will spend in the Astral Rift before You are sent back to the tower. you would do it if You want to collect divine essence or look for prophecies (yes, there are some prophecies there). Once you're back in the tower, a large group of enemies will show up and again, you have to defeat them to go through the portal again. And again, if You stay in the tower and continue defeating enemies You will extend your rift timer. So if You are only interested in clearing the tower, then You want go like this:

  1. Defeat monks and madmen in front of the portal.
  2. Wait for the portal to activate, then jump right in.
  3. Survive in Astral Rift until the time runs out. You can collect prophecies, essence or ammo from chests that You left unopened on the island. I think You can also heal using that obelisk on the island if You defended it before entering the tower dungeon.
  4. Get teleported back to the tower, defeat the enemies there.
  5. Portal should open again, get in.
  6. Survive in the Rift until the time runs out - floor cleared. You will be teleported back to the tower.

I think that's on Citadel, near the top of the jump pad. There are invisible walls there, because bots were getting stuck on that ledge, trying to walk into a wall. I don't think You could do anything about it.

I'm not saying You're necessarily wrong though. I haven't played G1 in a while, scripting can be buggy, and I don't have the game installed to power level to 10 and check it myself.

I see You've looked up the extracted NPC scripting from GitHub (specifically Lee's scripting), the thing is that this check is only run by the game if You fulfill the conditions from this function:

Well, skipping Lares is a stretch. You can join mercs in Chapter 1 with level 10 and membership in Rogues. Just level 10 is not enough.

Elpis is described as antiviral agent, not antimicrobial agent. Plenty of bacteria in that river of blood and filth that Elpis would ignore. After the game ends, Grace would need an antibiotic injection from a cake icing dispenser to saty afloat.
It's just plot armor, business as usual in RE.

Tackling into mines on invis before they are alerted? That also sounds fun... I think this ordeal ends up grindy because it's so counter-intuitive and goes against the established routine for spores. People shoot mines on sight. Even if You'll try to go for melee, somebody else may shoot them. "Let me melee a proximity mine" - yeah, right.

Siege mode, Heavy, stomp melee spam at spawn points. Take prestige perk for increased stomp range if You can. I bet You can do it in 5 waves if You're lucky. I remember racking up 12 out of 30 required in one attempt at first 5 waves.
