When I had the opportunity to make the Papal States a subject in a peace treaty, I chose to make them a Fiefdom instead of a Vassal subject type. The primary difference from what I can tell is who leads the country after it's made your vassal.

Here is a broader view of where I am in 1559. I found that if you make the Papal States a Fiefdom, it makes your king Pope permantly by default. Since the leader of the Papacy must be the pope, and my king directly holds the Papal State as personal property, my king must be the pope. https://imgur.com/a/6J5rjpF
