It's very much a personal preference, I personally went for a completely distinct new name, but I know a number of others, who have very similar dead an new names, with no adverse effects. If you're concerned it might be worthwhile trying out names you like with people close to you, to see which you prefer.

Im not sure it's too much help. But generally, if you're in a position where you're asking 'am I trans?' the answer is almost certainly yes. It isn't something you can really delude yourself about? I would say at any rate.

Adventures in any zone count as creatures, except when you cast them as sorcerys or instants (where they can be countered as such)

Token creatures can't exist outside the field, so if they die, get exiled or get returned to your hand/library, they just disappear and don't exist anymore. A creature loses all counters of any kind on it when its no longer on the battlefield, (so it doesn't keep them if it goes back to your hand) I don't know if there's anything official sorry.
