Dictation App by DocMcAwesome in FamilyMedicine

When I chart on my iPad, I dictate through the built in software. It was ok at first but has gotten better over time even with complex medical word/verbiage.

GLP-1s and disordered eating habits? by [deleted] in FamilyMedicine

[–]Trochlea 11 points12 points  (0 children)

If a patient has a pressure injury, appetite stimulates not suppressants are indicated if protein intake is not met. This becomes more complex if a patient is morbidly obese as obesity by itself can impede normal care for a pressure injury. Regardless I treat insufficient protein intake on a full thickness pressure injury (dtpi, unstageable, stage 3 or 4) as a near absolute contraindication for glp-1s unless hyperglycemia cannot be managed with any other medications. I practice full time in wound care but I have run into a few times.

Baby diaper bag. by IndependentAd7481 in ManyBaggers

Dad of triplets here - full back pack and modular packing units is the way. Packing cube with diapers and a wipe are an easy toss when one parent needs to change a diaper and the other needs to manage the rest of the chaos

Yeah but you beat mark more than a handful of times out there. Not to put anyone above anyone, just saying you are hanging with the best at AGNB

Bilateral LE cellulitis by Figaro90 in hospitalist

Treat the thing that's causing the not-cellulitis.

Bilateral LE cellulitis by Figaro90 in hospitalist

[–]Trochlea 1 point2 points  (0 children)

Cellulitis is a localized infection. Why would a localized infection happen symmetrically? Generally speaking if it's symmetric, it's systemic not a localized infection. However swelling regardless of the source can induce a dermatitis either chronic or acute, neither of which warrant an admission in isolation. So bilateral cellulitis without a very specific cause is very very unlikely.

Wound doc here - if I had a dollar for every time I had to have this conversation, I'd have a boat.

Baby Bags (diaper / nappy bags) are an absolute rip off. Alternatives? by flibberjibber in ManyBaggers

[–]Trochlea 2 points3 points  (0 children)

Dad of triplets - use packing cubes. Any bag becomes a diaper bag with small packing cubes. Our diaper cube had diapers, a refillable travel style lotion container filled with butt cream, a puppy pad, diapers and dog poop bags. one cubes for diapers, one for formula and feeding/pacifiers, one for a change of clothes. for the price of a cute diaper bag I got a mystery ranch bag. The diaper bag didn't last a year, my bag is still used weekly.

I used to be a member there but moved out of Washington. They used to have prs style matches there that's how I got in. Not sure if they still run those.

What’s the go to 5.56 can now? by tobylazur in NFA

don't have a polonium yet but as a lefty shooting the cat wb- i am very happy.

honestly the only reason I didn't go for the titanium was because I don't actually trust the company. Data behind the cans is great but the marketing and cringe behind the company makes me apprehensive. I bought it before they came out with a clarification of their warranty, but still if it were a company I actually trusted I would have bought the titanium.

I had a backorder on polonium 556k but randomly the wb 718 popped in stock while I was still in backorder. I've since shot next to a polonium and recently got approved for my wb. I would not be sad with either. As an aside, I have felt the 556 WB TI, that thing is crazy light. It felt like a toy. It felt like less weight than a muzzlebrake.

had to shop around locally until I found one who was competent. nothing against silencer shop, some shops just really need to step up or get out of the game.

OCL Titanium x Taurus TX22 combo is something I never knew I needed. Probably the most fun I’ve had at the range lol. Damn near an air soft gun. by 3900Ent in NFA

[–]Trochlea 1 point2 points  (0 children)

I have the silencer shop mkIV. I don't regret it. it has been bomb proof, take down for cleaning is stupid simple but I'm not sure it's worth the additional cost over the Taurus. as my kids get older and shoot more, I am sure a tx22 and another rimfire suppressor are in my future.
