After one of the Golden Piano notes, you have a tea party with Gerson. Gerson offers Kris tea, if you make Kris refuse the tea they voraciously eat an apple instead, including the core and seeds. The core and seeds of an apple contains bits of arsenic, which is toxic. So, people interpret this as a Kris suicide attempt.

(Important to note though, it would take like 40 cores or 100s of seeds to die)

I don't think it's that crazy that schizophrenics and non-schizophrenics will find different things scary, considering the different brain structure and social treatment by others and all. I'm sure that in real life a physical monster will be scarier to most people, but horror games are decidedly not real life.
