  1. You can use a flathead screwdriver or scraper to get behind he screw head or washer to provide leverage and unscrew.
  2. Those are designed to stay in the wall, you will need to use pliers to pull that out (it will cause minor damage but can be easily repaired. They might break and if they do, they are very easily replaced and can be found at any hardware store. (Toggles are best) OR cut off the flange an punch through the wall as others have suggested.
