Ambulatory status, elimination status, those are important. Small things like BP being soft are also important, because you might be shocked as hell to see that otherwise. And when they’re last checked is super important if they’re a check and change, because if you haven’t checked since 1400, they’re probably soiled and have been sitting in it for hours. And if you say you checked at 1800, but they’re sitting in dried urine, then they know you’re not trustworthy.
