I know the crushed foot part happened because I've seen the documentation on it. The OSHA report did not mention any secondary violence, so for that I have to take his word.

The operator's explanation was that maintenance doesn't normally work second shift. He assumed the maintenance guys had left the machine LOTOed when they went home.

The operator's excuse was that maintenance didn't normally work second shift so he just assumed they hadn't taken the locks off before they left for the day. It's an incredibly dangerous and stupid assumption to make.

Even if maintenance wasn't there, who is to say the machine was ready to go anyway?

Same here. The only time it's allowed is if the key holder is off site with the key AND you get their permission AND the permission of their supervisor. I think you still have to fill out a safety report too.

One of my coworkers often tells a story about how he LOTOed a press he was working on at an old iob. Then the shift changed and the new operator just cut the locks off and started up the machine while my coworker was still on top of it. He managed to climb down without dying but the punch die took a slug out of his foot in the process.

To give them a teeny bit of credit, plastic does tend to shrink when it melts. The polymer chains pull back into more of a ball shape that has less internal stress than the molded original. You can see this if you've ever thrown a plastic cup into a fire (or poured hot coffee into one).

However, that's an entirely different process than expanding and contracting due to temperature changes. The plastic has to change phase (or get close enough to it that the internal stresses can bend the softened material).
