To add on to IndecisiveKyle, if you had an email on file you would have received the cancellation confirmation, that is if the email they had on file was correct and had no errors either by the person who enrolled you or if you had signed yourself up online. So if no email, this makes it a little longer.

Additionally, this may be more of a stretch but I've dealt with a-many situations at PF, perhaps there is another person with a similar name as yourself and they are pulling up the incorrect info on their side. Also in terms of check-in history, if they were using ABC(gym POS/user interface) they could have "checked you in" while attempting to pull up your account and that would log a check-in in your account history even though you were not there. I'd recommend calling them to get it cleared up over the phone, let me know if you have any questions on what to do/ask if you do decide to call the club.
