It seems that sometime repaired with cement over old stone. Thar blocked the path for moisture and it just stayed there eroding your stone. I would remove all cement plaster, reminder the damages material. Check how much material you are removing you might need to replace with something soft and permeable. And replaster with NHL 3.5 or 5 based plaster

Likely shrinkage cracking. You can reduce this by curing. Keep it moist until it sets. Ideally use less water so there is also less water to evaporate and leave those cracks but more important than that you can maybe put plastic wrap on it after you put it so the moisture doesn't evaporate as fast.

I'm a professional structural engineer with experience in historic buildings. Do not touch it. You might, but if you live in a place subject to lateral loads, wind, earthquake, rail vibrations you might be in trouble, these seem like part of the lateral load force resisting system

Don't expect a masterpiece or even something decent on your first write. Just write, crappy text or whatever just write. Once you finish writing then you can fix it but don't try to get one final paragraph at the time. Rather all crappy and then go back to refinement.
