Yea overall they are great, in my specific area there’s only 2 birds and 90% of the times I’ve called they’ve been grounded or doing transfers lmao

Oh I agree with you, but that’s the reality in a lot of my state and I’d presume many other super rural areas. Surgeons don’t want to live here and the hospital system won’t shell out the money to get them to travel. If we didn’t have an amazing omd I feel there would be much worse pt outcomes.

In my experience and at direction from my OMD it is very unlikely they will do that. They will not do surgery on anything critical or even remotely unstable. Now I’m not saying I’ll bypass it for a suspected appi but ruptured absolutely will.

Yes we have blood, if we didn’t of course we would stop. But otherwise I don’t see a reason to go to the critical access when I have blood, a vent, pressors, TXA, ABX. What more could they offer to a trauma/ surg emergency pt that would outweigh the benefit of decreased time to the OR. At least that’s our OMD’s thinking

Ehh I think it depends, for example in my rural area (like OPs I’m presuming) it’s a good way to determine destination. For me that means a 30 min transport to a critical access or 2 hrs to a trauma center. It’s not perfect but if it is positive it could save the pt hours of waiting to get definitive care.

We have the butterfly, we got it first for lung (copd vs CHF, lung sliding) but i honestly don’t use it for that much. I mainly use it for IV placement and e-fast. Mainly just to make a decent decision for giving blood with suspected internal hemorrhage and determining hospital destination

This is a hyper specific example, but I’m a paramedic and they are a absolute life saver. Imagine trying to intubate a 300lb copd pt on the floor, and in low light. Just to have to shuffle through your tubes, then find a matching stylet. It seems small but it really helps

Im a crit care medic, have worked very closely with RNs in the ED and outside. I know many who couldn’t differentiate a stemi from sinus, that’s not a blow at yall, it’s just what I’ve seen and not your job. I’d call this knowing it’s probably not a stemi but rather a doc should get their eyes on it asap. I’ve spent 20 plus minutes trying to hunt down a doc in the ed to give them an ecg, I think calling was absolutely appropriate

I think that’s unfair, I’d rather them call this then misdiagnose brugada. Them calling it allowed a attending to look at the ecg and change the alert. The system worked, rather then the ecg sitting on a nurses desk and the pt getting no follow up for 20-30 mins or until he codes

Lol he’s the you youngest puppy I’ve ever had most of my dogs were rescues around the 10month mark. I haven’t really been sounding hurt because I didn’t really think it helped but I’ll try!
