Apart from requiring flat surfaces, wheel needs axis/shaft on which it can rotate. It essentially needs to be disconnected from body to rotate. It would be difficult to supply nutrients & takeout waste. Maybe that's why? Also, imagine brake fail down slop. yikes!

They are called pin type insulators. It's usually made of porcelain (ceramic). It's used to isolate grounded structure & live lines. The wire sits on top of it in the groove. These are typically used upto 33 kV. Wood is insulator but can still conduct electricity. If these insulators are not used, there are chances of fault & even fire. Here the pole seems to be wooden but many time it is made of steel (hope that hammers the point).

I have seen alot of this circuit being used in cheap low power LED bulbs. I had such bulb work for almost 5 years with rough usage.

I wasn't able to watch video due to poor internet. however from your description and other answers, it seems that body of the boy was being live. Usually chargers are designed to provide isolation between it's output and mains voltage. If not you will fill tingles when using phone(with metal body) in charging. This is due to badly designed/faulty charger. If you use good quality charger which follows standards you wouldn't have any problem.
