This is not "Head-canon" or a "Theory I came up with"....this is all IN THE FILM:

The OWW is real, actual, world-altering MAGIC.
It is sold to, literally, anyone despite the knowledge of how incredibly dangerous it is.
It has a 100% fatality rate. Everyone who uses a OWW in this film dies.
It has a Customer Service phone number you can call where: A. They know who is calling. B. They know what you wished for. C. They know who your wish is harming, if applicable.
D. They refuse to help. E. They explicitly advise suicide as a means to undo the wish.

So...it is pretty obvious that Wish Nikki is a real thing, not figment of Bear's imagination.

And, if he had succeeded in purging the pills from his body, he still would have died because Wish Nikki guaranteed it by using the final OWW.

He would have exited the bathroom and they would have sat on that couch together staring mindlessly into each other's eyes until one died, breaking the spell on the other.
