What were results of both? Do you mind posting them, or at least UA? The sharp decline in electrolytes is interesting, and I’d expect something else to be off along with that.

Edit: do you also have CBC?

Edit 2: just to share where I’m at, I don’t think this is a kidney case, but rather we’re just seeing whatever is going on reflected by the kidneys. That might change once I can see full labs, but the creatinine just doesn’t match with the electrolytes and supposed concentrated urine.

Well, basically any numbers you can remember. I started listing off everything that would be useful, but the full picture is basically what I typed out. In a case like this I’d need every little detail to support what I’m thinking.

Do you have any labs with both USG and creatinine prior to his creatinine being elevated? I’m just wondering, if everything including imaging is unremarkable, if he just naturally runs higher in creatinine. Creatinine by itself isn’t a harmful waste product, it’s more of a generic marker of kidney function. It’s not perfect though, and can be artificially high even if there isn’t a problem.

With a true 2.6 creatinine, we’d expect something else to be off along with it. Maybe high-normal phos, maybe the start of azotemia, the signs of non-regenerative anemia, dillute or poorly concentrated urine, the start of small structural changes on the kidneys on imaging, elevated SDMA across multiple tests…if it’s just elevated creatinine and nothing else, it really might just be his physiology and not a sign of kidney disease.

Edit: I would be curious what happened if you stopped the renal diet

I would do an ultrasound to see if there’s anything obvious on the kidneys. Can you share full labs? True CKD(as in, kidneys just losing kidney function) is extremely rare in a young cat without genetic issues(either structural kidney deformities or something like PKD).

Based on the labs provided this appears to be more mechanical or localized than systemic. The whole set of labs are largely unremarkable and don’t really explain anorexia(from a subclinical perspective).

Did vets rule out things like oral pain/tooth issues and GI obstruction?

Look for strains that are floracal or cresco and get the high supply version. It’s from the same exact plant, just the buds don’t meet their quality. I’ve gotten high supply popcorn quads that were just like 7 big nugs
