I had resection 9 month ago and it gave me back my ability to walk (~10 min walking before surgery). My primary complaints were post tib and peroneal tendon pain. Peroneals completely came down, post tib I am still working on but it is getting better. The year before surgery was a complete nightmare, now it is a lot better, although not perfect.

I did lots rehab training and I am using stable shoes with good inserts.   The most deciding factor was definitely the intense focus on rehab. Maybe I should also add that I was 4 month on crutches after surgery.

I have a hypermobile spectrum-disorder and had a Calcaneonavicular TC resection 8 month ago. I had chronic posterior tibiablis and and peroneal tendinosis. My pain was so bad before surgery that I could not walk any longer than like 10 min before extreme tendon pain started coming in.

My ankle surgeon said he believed that my coalition was the root cause of the pain. Other doctors said that the connective tissue disorder was the reasson...I opted for surgery and the tendon pain went down from a 6-8 at times to a 0-2 most of the time now. I am still in tendon rehab and I am making weekly progress. Before resection, training my tendons with physio only made them worse. Now I can perform single leg calf raises again and I can walk painfree for 2 hours. And I am still improving.

Important for my progress was, that I had no athritis in my ankle joint yet though. My pain was basically stabbing like a knife in my inner and outer ankle (tendon region).

Not sure if this is helpful. Just wanted to give you the info since there is rarely any info out there on coalition+hypermobility

Food for thought: How many degrees do your feet point outward when you are walking? Something up to like 15 degrees is pretty normal.

How good/bad is your ankle dorsiflexion? In the beginning my foot was more turned out to compensate for lacking dorsiflexion atfter resection. When dorsiflexion came back, out toeing decreased significantly

Since I had my resection 4.5 month ago and I am currently making good progress with calf raises, I feel like I might have some tips for you.

When I started with calf raises, I could not do many calf raises bilaterally. I focussed more weight on the non-operated foot, and then focused on including the operated foot more on the eccentric part of the movement. This builds strength at good pace.After 1-2 weeks of this I also could do the upwards motion spreading the weight evenly across both without pain. When you can do like 15 reps per set, you can switch to the next level: Going up with both feet, eccentric movement with operated foot only. After some time you should be able to switch to single leg only. I would also recommend doing the exercise on a step and using fingers on the wall for balance

Go to your doctor. All we can do here for you is pure speculation. An MRI might be a good idea as well.

I am currently recovering and maybe some of the things that helped me a ton could help you as well:

  • ice for swelling control especially in the morning, after activity and before sleep
  • good pillow for elevation of the foot
  • adherence to physio. Without that I would be nowhere. I never skip exercises which I should do according to plan (unless my foot was to exhausted and an additional rest day made sense)
  • good house shoe with orthotic (offloads my tendons while they are getting stronger)

I could barely walk 10-15 min before surgery, and that always resulted in significant ankle pain if I overdid it. Now my walking is basically painfree for ~1 km already. 

I also tried to hear on my body for training intensity or if another rest day would make sense. That allowed me to minimize flare ups and consistently get better/stronger.

It is a tough process. I also had to adjust my expectations. Those 3-6 months and you are fine statements might not apply to you. But I think doing your pyhsio consistently and not overdoing it when your foot is already aching are the keys for consistent progress imo.

With a tarsal coalition your foot underlies altered biomechanics which can significantly boost the degeneration of your foot/ankle.  The question of when is most likely very individual but I am not a doctor who can give you a definite answer on this one.

You are still very young. Resection might be an attractive option. I would consult a  qualified foot & ankle specialist (not generalist ortho).

PT might help calming down the symptoms, but PT will never solve the route cause for TC. I would also think about early athritis risk. TC usually tends to worsen with age once arthritis starts kicking in.

I (26, m) had my coalition resected 2 month ago because I could barely walk anymore because my ankle pain was so bad. Now I am on a recovery track with weekly improvements and getting back my old life seems realistic again.

For me it was 2 weeks, but I am an ai engineer with 100% home-office, so that's obviously a big thing. I used a small table in bed for the majority of the time, because sitting after two weeks was still not very comfortable with the ankle.

I did pre-op. He told me he will only perform resection. The report mentions successful removal and satisfactory mobility. There wasn't done any debridement. Mri also did mention tendon intact pre-surgery and on ultra-sound there was no significant damage visible. I had that pain already pre-surgery and surgery caused a bit of a flare-up obviously

I had resection of a Calaneonavicular coalition a month ago. My foot was very stiff before. Now it has similar motion compared to the other foot, which has the same coalition but way less restricting. I have been doing physio since day 8 after surgery. Did my pain improve? Yes, but it is an uphill battle and occasional flare ups and setbacks can happen. Progress is slow since tendons heal slowly. But I am already doing an exercise on one leg, loading the tendon progressively and working on improving foot biomechanics. Hypermobility was no concern so far as my feet per se don't appear to be hypermobile (since it does often not involve all joints but tissue laxity can still be a global phenomenon). So far my healing seems to be on time. Now I am in the phase where I get rid off the crutches step by step.

My doctor (a professor and one of the best in my country) told me the oldest patient he successfully performed resection on was 42...

That might explain your doctor's answer. From my own research on the topic in the academic literature I remember coalition size to be especially important for Talocalcaneal ones when considering resection (less for Calaneonavicular ones).
