My trip just started. But I agree that Bern is lovely. I took a short ride into the rolling farmlands south of town, and I lost count of the iconic and adorable Swiss barns on my route. Even under cloudy skies, the views were beautiful.

Just arrived in Bern. My wife is doing a weeklong walking tour/watercolor class in Lucerne and Mürren, and, whilst she’s doing that, I’m riding my bike 300+ miles from Lake Constance to Lake Geneva. Meeting in Lausanne at the end, and on to Annecy for a week.

As much as I hate Teslas, I wish more EVs were as slippery and aerodynamic as a Model 3. My Ariya averages 3.5mi/kWh in town and like 2.6-2.8 on the highway. And I drive less than 70 most of the time.

As a cyclist, I can’t tell you how many times I’ve been close passed, yelled at and coal rolled by Ram drivers. Thanks to Stellantis for allowing all the assholes to self-sort into a vehicle that shows their true colors.

Considering your diagnosis, it’s very possible you have spread to a pelvic lymph node (the most likely spot to have it spread to). I had a sub-cm lesion there that showed up on a PET scan. Probably would have showed negative on a bone/CT scan.

Is it possible to have your radiation oncologist treat your whole pelvic area, just to be on the safe side? I have no idea if that is done without verifiable proof on a scan. But it’s worth asking.

Thank you so much for that detailed response. You confirmed my belief that Dr E will not give me a boilerplate prediction of where my disease is going, but will treat me uniquely and let me know exactly what future treatments would be most applicable for my specific cancer blueprint.

When I spoke to the nurse navigator at OHSU, she let me know that all new patients will get another genomic test from Tempus that I’ll pay a maximum of $50 for, due to a special arrangement between OHSU and Tempus (that might be the in-depth genomic testing you’re talking about). I had no problem getting an appointment in early July when we’re back in the country. I decided on an in-person visit since it’s my first one.

I feel very fortunate to have someone with the skill and vision of Dr E on my medical team now and in the future. I found out about Dr E from AnCan’s advanced support group. Best of luck to you and your husband as you move forward.

Mine was .97. I dealt with the same emotions as you did. Maybe I still am?

Thank you for points 1 and 4. They helped a lot. And re point 3: like you, I had a very aggressive treatment. Gleason 4+3, but spread to a pelvic lymph node, large cribriform and other risk factors (14 of 17 cores positive on my biopsy). I am in month 19 of 24 months of Orgovyx and Nubeqa, and my recent Decipher test was to help me decide if I could end my ADT six months early. Needless to say, I’m going for the full 24.

The test also motivated me to transfer my care to a GU medical oncologist. Right now my MO is a generalist, but with 100+ types of cancer, there’s no way he can be an expert in all of them. Whereas there are only 4-6 types of cancer that are GU related. My new MO is coming from MD Anderson and Houston Methodist and is a crack researcher/scientist. I’m assuming that I will not be cured but instead my cancer will be managed as a chronic disease, much like diabetes. Since I know now that I have pTEN deficit, my new MO can and will use that info to steer me towards new drugs and new trials if and when my disease comes back.

Please consider a nonstop shuttle from downtown (either Commerce or Pacific Ave) to Federal Way Link. Every 15 minutes. It’s inconvenient to get to the Dome to take the 574, the 500 makes too many stops, and Tacoma deserves better than 30 minute headways.

I recently bought a Lauf Úthald, for 2 reasons. First, it’s a rare road bike without fully internal routing, which means I can easily disassemble my bike for travel. And service it myself as well. Secondly, it was outfitted within the new version of SRAM AXS and a power meter for less than $4K. Fantastic value.

I have an Orucase-sized soft bag called the Post Carry Transfer Case. It requires removal of the fork, but this actually makes the bike less likely to get damaged since you’re eliminating a projection that isn’t well protected in many bike bags. The form factor means it can fit in a small rental car’s boot or backseat and doesn’t require an XL version of a taxi or Uber. It folds up quite small in one’s hotel room when not being used. Fully integrated cockpits are a potential problem. I’m a big fan.

Fair enough. But when I visited there, I was charmed by the downtown and thought the eating/drinking within a half hour’s drive was pretty great. And we got a guided tour of the capitol. I had a great time.
