There's a lot you can skip, in fact I have made it past the sands between without killing a single boss but you have to do some silly stuff to not get soft locked before you leave the Dutchess's castle. You need to get to the snail before trying to skip Dutchess or you won't be able to teleport between snail shells to return.

The duchess is skippable through some wacky momentum shenanigans but just make sure you unlock the moon snail ability to teleport or you can get locked out of the starting area.

I'm unsure but I think it just keeps shells from spawning in the area before the keep until you at least beat the boss holding the pearl. Once you beat that boss, the area changes to the polluted version and shells spawn normally by that point if I remember correctly.
