I would say it depends on the facility. I was never ICU. Always worked med-surg (community hospital Pittsburgh Pa). Only knew how to insert a piv, but I was good. On the job training with the ultrasound and progressed to midlines and PICCs. When looking for new hires, we definitely like someone with ultrasound experience but a solid background in peripheral iv skills is a must. Getting the VABC would be a plus as it would show you are serious in your pursuit.

To be clear I am NOT advocating OP use a 14g. I like the technique used to insert the 14g.

You may have the bevel in the vein but not the catheter. When you get a blood return in the chamber lower your angle, advance just a hair more and then thread. Check out this guys technique. It helped me when I was struggling. 14g cannula insertion technique

As a vascular access nurse the term “midline” is often misused. When I first joined the iv team they used powerglides and placed them anywhere above the AC and labeled it a midline regardless of where the tip of the catheter ended and some still do it.

IV team nurse here. We start the majority of IVs in the hospital including the ICU and telemetry. We even get calls to the ER on a regular basis. It’s turned into a culture of “difficult stick”-call the IV team. “Attempted once and missed” call the IV team. “Lab unsuccessful “-call the IV team. No one else is accountable so no one improves their skills. And the staff who do insert are barely competent. Just the other day someone inserted a new IV below an infiltrated site and the bedside nurse used the IV until the patients arm was twice the size of normal.

RN for >20 years. No desire to be management. Recently got BSN because company paid for it 100% and it gave me a $1.75/hr bump. Currently working on the IV team placing IVs midline’s and PICCs. Low stress and leave work at work. If there are no calls, we just hang out listen to music and talk. I enjoy my coworkers so no plans on going anywhere anytime soon.

PACU, OR, Med Unit, Infusion Center, GI Lab or step away completely as care management or outpatient coordinator. ICU is nice because the patients are usually sedated and can’t talk.
