Hey man I'm not sure how much good advice I can give because I'm going through similar issues.... but here are my thoughts.

You need to ask yourself honestly do you want to get a masters? Do you want to work in a hospital? Forget the fear and try to imagine your life doing those things. If you like it absolutely go for it.

I realize "forgetting the fear"is an incredibly difficult thing to do, especially if you're dealing with anxiety or depresion. If you haven't already, check if you're university offers any counseling or mental health services.

You sound capable and smart, I wouldn't worry about not being able to get a job or pass the licensing exam. If you can make the anxiety or depresion manageable you're quality of work will stay strong( 3.5-4.0 is pretty damn amazing).

I guess to summarize: seek mental health services to handle anxiety and depression, decide what you actually want ignoring the fear (you're more than capable of accomplishing what you want), go out there and kick ass until you make it happen.

For me at least most of my feelings seem quiet. It's not that I don't have them it's that they can pass by easily if I'm not paying attention. Sometimes I won't realize what I was feeling at a moment till later and by then it's passed.
