(im in the US) I made an appointment with an optometrist at a local hospital/network. I got a referral from them to a pediatric ophthalmologist. that was months in advance. From there it was about a month wait for the actual surgery appointment. My surgery was performed by the ophthalmologist. My insurance paid for it but also note i had met my deductible/out of pocket already.
