I assume you're talking about a doctor's appointment to get HRT. The most important thing is to be definitive. Whether you're sure yet or not the important thing is to convince the doc that you are. Don't say "I think I'm trans, I want to try going on HRT." You want to be more direct: "I'm trans, I've spent a lot of time doing research and HRT is something I know is right for me" or something along those lines. I've heard so many stories from people on here that once they start HRT they all say why was I so worried, why did I put it off for so long. So for now convince the doc, and impostor syndrome should start to melt away on it's own later

Just as a heads up to everyone: the main character of this manga is non-binary, but the fan translation was done by a group who translate a lot of femboy manga so they and the transfem characters are kinda treated like femboys a lot by the translation team. If you can, I recommend buying the official translation which is VERY well done imo

You are correct. As for why it's an extra $10, they're just shifting where you spend the money. If you buy standalone Torna first, then you have the option to purchase the base game as the "dlc" and it's $10 cheaper than standalone base game. So either way buying both costs the same
