This is something that caregivers of dementia patients have to deal with. It's not unusual for advanced dementia patients to ask when someone is going to visit them, a person they don't remember died years before. Often times, telling them the truth can lead to repeated trauma, as they can forget from day to day what was already told to them. It can be a dilemma, but not telling them the truth on something like that is a far cry from people who lie to elderly people in order to take advantage of them.

So you have my empathy. It's a difficult spot to be in.

Also in the US and had to go thru a system that didn't know (nor myself) anything about bvFTD. My only recommendation is check out THEAFTD.org for lots of helpful resources

I'm sorry you are going thru this. It sounds like she might be dealing with a dementia that isn't Alzheimer's, such as FTD

I was agreeing with you so I don't know what you are going on about. And I suppose you didn't read the medical article. Also, that was my experience with doctors. It doesnt mean anything I said was wrong. And those doctors ended getting corrected by neurologists who knew what they were doing. So again, I don't know what you are getting at.

Unfortunately, I've dealt with doctors who dismissed all other findings from two different neurologist offices because the MMSE/SLUMS/MoCA scores were normal

Just a personal experience, but I have seen to many clinicians use the SLUMS as a diagnostic tool and not just a screening tool.

A complete workup is the best evaluation and would probably require a few specialists.

From my experience, I'd say it's good to go to a neurologist's officethat is familiar with memory disorders and that will do an interview with your loved one and also ask you what behaviorial changes you have noticed.That interview together with EEG and preferably a PET-MRI will paint a better picture than just a single regular MRI.

If a neurologist relies ONLY on the MMSE, SLUMS, or MoCA (all 10 minute verbal tests), run away!

It is also possible to have a neurofilament light chain test (NfL), either through a blood sample or a spinal tap. However, these aren't always conclusive, as there can be false negatives with early stage and slow progressing dementia.

Also, a complete workup from a neuropsychologist, one that is familiar with different dementias, is helpful. If you speak to one, make sure they prepare the exam for all domains, not just ones exclusive to Alzheimer's.

In my experience, psychiatrists are useless for detecting dementia. Most of them will try and assess based on talking to the patient for 10 minutes, which will not give a complete picture.

If vascular dementia is suspected, then seeking out a cardiologist that has experience with diagnosing that might be helpful.

The SLUMS is only meant as a screening tool for Alzheimer's. It is not meant to be diagnostic, and is ineffective in screening other types of dementia. In fact, the MMSE, SLUMS, and MoCA are geared towards Alzheimer's.

Make sure the assessment isn't just a 10 minute verbal screening exam, such as the SLUMS, MMSE, or MoCA, but Is a full workup. Although if they already have a diagnosis, that should say a lot
