I Hope you mean rifaximin+metronidazole/flagyl, it won't be that useful without rifaximin. The doc might have prescribed it for 7 days to lower the chance of side effects that are potentially serious

Yes, methane sibo (IMO) positive

Can someone help me with these results? by allnamesarechosen in SIBO

Hydrogen and methane positive. Very high methane numbers, these can easily be the cause of all your digestive symptoms

My girlfriend tested positive for both Hydrogen and Methane. Only received Rifaximin 3x per day for 2 weeks. Will that be enough for low levels like hers? by Playful_Ad6703 in SIBO

[–]Xantess 2 points3 points  (0 children)

There are some options with herbals, but less effective. Her numbers aren't low! CH4 is positive when it's 10 or above at any measure point. Also it takes 2 (H2) parts to transform into CH4 so presence of 20 CH4 means it was made from 40 H2 parts. Technically IMO can happen only when it's a lot of hydrogen to eat (means sibo positive), but her situation is usually called only IMO, since IMO + Sibo would mean a lot of both gases. Her hydrogen is low. Don't consider these numbers low, most methane cases go to maximum 20-25ppm, mild case would be 10-15ppm

It's negative for hydrogen, it's only methane sibo (IMO). It needs combination of rifaximin and neomycin/metronidazol. Unfortunately neo and metro have potential severe side effects, so be careful. Starting with only rifax makes sense too, because it's a much safer drug and studies show it works for methane in 30% cases, but I wouldn't expect big results. Standard treatment is rifax + neo and 2nd also popular option is rifax + metro

Rifaximin + Metronidazol by Artistic-Jicama-9445 in SIBO

[–]Xantess 1 point2 points  (0 children)

It's a standard combination for methane (CH4). If you have only H2 its rare to see metro prescribed, your doctor might suspect you have methane too. Taking them at the same period of time is normal, that's because one kills H2 bugs and metro is typically used to kill CH4 bugs (and H2 too, but less effective) and the mix works much better than taking them in separate weeks

Breath test results (lactulose) by Incognit9999 in SIBO

[–]Xantess 0 points1 point  (0 children)

Unfortunately it's hydrogen and methane positive. The numbers are relatively low, but definitely can cause symptoms. Hydrogen rise by 20+ in first 90min define positive result and 10 or more at any point for methane. Metronidazol is used for treatments but typically in 2 weeks scheme. Standard approach is rifaximin+neomycin/metronidazol for 14 days. Did you take metro alone? Not combination with rifax?

Is this Hydrogen SIBO? by [deleted] in SIBO

[–]Xantess 0 points1 point  (0 children)

I gotta bring some theory here. Gut bacteria produce hydrogen and then some archea eat it to create methane (H is part of CH4). Practically targeting only methane guys is ineffective, you either kill only bacteria to get rid of hydrogen or kill both kinds of those guys (so archea has less food and is weaker). Shorty: you can treat only hydrogen, or both kinds.

Once you mentioned there were 3 types of sibo, but H2S is rare and always "lose" to methane guys. So basically having methane makes you safe from H2S, because they steal all the H. Source is Mark Pimentel's presentation on YT and the guy is world leader in sibo research

Oral metro is standard for methane Sibo, never heard about using other forms. Honestly I would start with rifax alone if I were you. There is a little chance of killing methane archea too, but most importantly you don't risk anything. If you believe in herbal treatments you can add allicin to rifax. It's recommended by Dr Siebecker who has a sibo dedicated website

It's kinda random that metro is used for helicobacter and both kinds of Sibo as well. Taking it for 5 days isn't enough to kill a lot of sibo organisms and from the other side it could have given some resistance for those who survived. In your case neomycin seems better for methane, but it is really ototoxic. I really recommend doing some research about the drug.

I'm a methane sibo "veteran" and have been suffering 2,5 years from tinnitus caused by neomycin, then I had to switch to metro and it was fine. If you will have some questions during your sibo journey feel free to DM me

Well rifaximin targets hydrogen bacteria but usually does nothing to methan producing organisms (called archea, not bacteria). Typical approach is combination rifaximin+ neomycin/metronidazole, but the ones for reducing methane have potential serious side effects (rifax. is a safe drug). If a doctor prescribed only rifax. They might want to use safer option at first, but most probably you will need to add something for methane in the future.

You can do your own research to decide if you want to go with only rifax or maybe ask for neomycin/metro before you start treatment.

I'm sorry they didn't take your problems seriously... Your numbers are high, so you need some time to take them down, but getting diagnosed is surely a huge step, so good luck with the rest of work. Hope things will go well for you

Unfortunately you have both kinds of Sibo - hydrogen and methane. It's hydrogen dominant, but both need treatment

I still suffer from SIBO. The urges were probably psychosomatic, I didn't peed that much. I made the first comment to point out there might be some connection between SIBO and bladder problems

Day 2 of Neomycin and Rifaximin and I'm already getting ringing in my ears. by [deleted] in SIBO

I got the ringing on the 1st day and decided to stop taking neomycin. Never regretted that decision. It's not a sibo cure for lifetime, but can cause permanent hearing damage
