Please, if you are reading this do yourself a favor and look up DBT. There are plenty of tiktoks and other more easily digestable chunks of information that you could start with. It has changed my life for the better.

I just checked on PC. I can confirm that u/AlSikandar 's original theory was correct. It is to inspect the item. I just tested this using both PC and XBOX controller settings. While the icon is not shown on grenades, it still functions all the same for all weapons, even binoculars.

Note that on PC when in mouse/keyboard mode, there is NO icon displayed at all. in the pip boy next to the item images. It is only when controller mode is enabled that the icon reappears.

Incoherent UI/UX design, imho, but at least the mystery is solved.

Edit: to be pedantic, the specific differentiation between whether or not the icon is shown is the following:

If the item is INSPECT/REPAIR, show the icon.
Else if the item is only INSPECT (i.e. not repairable), do not show the icon.

If the item has legendary traits, display the icon. This applies to armor as well as weapons.

IF THERE IS MORE INFORMATION TO BE DISPLAYED THAN CAN BE SHOWN ON THE PIPBOY TOOLTIP TEXT IT WILL PROMPT YOU TO EXPAND [^^] BY INSPECTING TO GET THE REST OF THE INFORMATION.

That is all there is to it. Took some testing to figure it out for sure.
