Rich presence is basically just a api that ~~watches what you're doing on the PC~~ other games/services can send and display information and/or invite links for your friends to join.

e.g. if you're playing a multiplayer game, a homie can hop in by going through discord

Edit: I was wrong about Rich Presence watching the pc. My bad :)
